Reports to: Director of Services, Associate Director of Services, HCBS Supervisor (dependent upon program
structure and location needs).
Supervisory Responsibility: None
Summary/Objective: The Direct Support Professional II is responsible for assisting in the implementation,
coordination, and documentation of goals and objectives for individuals served, as directed. The Direct
Support Professional II will be an active advocate for individuals served with an emphasis on individual choice
and decision-making. The Direct Support Professional II will assist by providing day-to-day leadership in the
provision of services including training of Direct Support Professionals.

- Provide input to leadership for the development of individual goals and objectives.
- Provide services based on individual goals and objectives.
- Comply with all applicable policies, practices, and laws, and with the rules provided by any governing bodies, such as Department of Human Services (DHS) and Centers for Medicaid and Medicare Services (CMS) and HCBS and/or DIA rules.
- Teach individuals rather than doing for them.
- Use behavior strategies to support individuals with challenging behaviors.

- Encourage and carry out recreational and/or community activities to meet needs and choices of individuals served.

- Complete incident/accident and behavior reports accurately and promptly.
- Provide for the safety of the individuals served with the least amount of intrusion as possible.
- Complete duties and tasks as assigned per shift.
- Respect the property of individuals served (e.g., care of clothing, wheelchairs, and personal items).
- Be an active advocate for individuals receiving services to have greater control over their lives to encourage individual choice and decision-making.
- Complete documentation in a timely manner following service provision. Turn logs in daily.
- Complete incident/accident and behavior incident reports accurately and promptly.
- Follow established protocols to de-escalate situations involving individuals’ verbal and physical aggression by utilizing CPI techniques.
- Teach and assist with routine personal care of individuals, including bathing, shampooing, hair combing, nail trimming, shaving, brushing of teeth, and dressing. Ensure the neat and clean appearance of individuals served, taking into account the personal choices of the individuals served.
- Teach and assist individuals with daily living tasks, such as room straightening, general cleaning, and laundry.
- Assist with individual program plans and the implementation of program of care, including the development and maintenance of individuals’ optimum level of functioning.
- Assist with providing transportation to individuals as indicated by their individual service plan.
- Ensure individual medications are administered, stored, and documented appropriately according to the six rights of medication administration.
- Ensure all service documentation meets Iowa Code standards utilizing the Edoc system.
- Include individuals name, date of birth, medical assistance number, day, month, and year, start and end time (including a.m. and p.m.) of service provision, as well as staff printed name, signature, title, credentials, location of services, change of service location and timeframe, and note of availability for health and safety.
- Ensure documentation includes attention to detail (e.g., completed legibly, errors corrected properly, use of approved abbreviations, cross out unused portions of log, etc.).
- Complete service documentation to record interventions and supports provided and reflect the goals and ongoing supports of the individuals served.
- Complete service documentation to record individual responses to interventions and supports.
- Scheduled work shifts:
- Find a replacement if unable to work a scheduled shift.
- Follow the department call-in procedure.
- Work weekends and holidays as scheduled.
- DSP II additional duties (as appropriate per region/site/program):
- Assist with the orientation and ongoing training of Direct Support staff as directed by the supervisor and ensure documentation of such via orientation training checklists, etc.
- Work with staff development team to stay up to date on effective teaching/learning strategies.
- Assist with the review of service documentation as directed by supervisor
- Ensure consistency of services for individuals served by working cooperatively with necessary releases and with other program areas or service providers (i.e., vocational, home health, nursing, case management, etc.).
- Communication via email/phone with HR and Staff Development regarding DSP staff training needs.
- Attending periodic meetings and/or trainings pertaining to training needs for the organization.
- Schedule physician appointments as needed and ensure appropriate follow up as directed by supervisor
- Ensure there is an ample supply of necessary forms at the site and that they are current.

Required Qualifications:
- SCL – minimum 16 years of age. HAB - minimum 18 years of age.
- HAB – minimum high school diploma or GED equivalent.
- Background checks with previous employers, references, law enforcement agencies, and others will be conducted and found satisfactory.
- Valid driver’s license and driver’s record deemed acceptable by Imagine the Possibilities, Inc.
