We are looking for an MMA (Managed Medical Assistance) Care Manager. About the Role:
As an MMA Care Manager, you will coordinate a comprehensive range of care activities for the members on your caseload, ensuring the efficient use of resources and facilitating access to the care required to enhance their quality of life. You will also conduct health risk assessments to identify needs and gaps in care, collaborating with members to develop personalized care plans that address these needs and support them in achieving their health goals.
Qualifications:
- Bachelor's degree in social work, sociology, psychology or a related field
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed to practice in the state of Florida
- Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities and or disabilities in a case manager or similar role or,
- Minimum of 2 years of relevant experience working with medically complex children
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Bilingual in English and Spanish is preferred
