Innovative Health Care Concepts (IHCC) is seeking energetic, self-motivated individuals for our Targeted Service Coordination program in our Pocatello office.
This is a professional level position working closely with people with developmental disabilities. Our current opening is part-time with an opportunity to grow into full-time.
Applicant must have at least a Bachelor’s degree in a human service field. Previous experience working with individuals with developmental disabilities is preferred but not required.
Job duties include:
· Monitoring and coordinating services;
· Developing service plans and assessments;
· Participating in team meetings with other providers;
· Documenting services provided.
