What are the responsibilities and job description for the Targeted Service Coordinator position at Innovative Health Care Concepts Inc.?
Innovative Health Care Concepts (IHCC) is seeking energetic, self-motivated individuals for our Targeted Service Coordination program in our Idaho Falls office.
This is a professional level position working closely with people with disabilities. Our current opening can be full-time or part-time with an opportunity to grow. Hourly wage is paid per billable hour.
Applicant must have at least a Bachelor’s degree in a human services field. Previous experience working with individuals with developmental disabilities is preferred but not required.
Job duties include:
· Monitoring and coordinating services;
· Developing service plans and assessments;
· Participating in team meetings with other providers;
· Documenting services provided.
