Job Summary:
We are seeking a dedicated and skilled Endovascular Nurse Practitioner to join our team under the direct supervision of the Department Chair of Endovascular Surgery. The Nurse Practitioner will be responsible for the primary health care of patients within their area of expertise, as well as providing specialty care under the direction of the medical staff. Responsibilities include conducting clinic visits, educating patients and nursing staff, and performing rounds at nursing homes, all while evaluating vascular studies and maintaining strong relationships with nursing home facilities to facilitate new patient transitions.
The Nurse Practitioner will plan and implement health care goals, which will be reviewed and confirmed by a physician, ensuring comprehensive care for patients and their families. Collaboration with the clinical nurse manager and staff is essential to meet patient needs effectively. Qualifications:
- Graduate of an accredited school of nursing Master’s degree in Nursing required
- Current RN license in the State of Michigan required
- Current APN license in the State of Michigan required
- Certification as an advanced practice nurse by the appropriate certifying organization required
- Five to seven years of clinical experience, at least three in the specialty area of care preferred
- Ability to maintain a high degree of accuracy in carrying out work duties
Job Duties:
- Conduct patient assessments and develop individualized care plans.
- Monitor and document vital signs and changes in condition.
- Administer medications and treatments with safety protocols.
- Provide direct patient care, including wound care and IV therapy.
- Collaborate with healthcare teams on care strategies and outcomes.
- Educate patients and families on diseases, treatments, and medications.
- Create educational materials for nursing staff and conduct training sessions.
- Assess patient education needs and develop tailored teaching plans.
- Encourage patient participation and self-management skills.
- Participate in daily rounds with interdisciplinary teams.
- Facilitate communication between patients, families, and providers.
- Document key patient information and address concerns promptly.
- Collaborate with social services for discharge planning.
- Monitor and evaluate care quality, implementing improvements as needed.
- Collect and analyze patient outcome data and participate in quality assurance meetings.
- Assist in developing and updating care policies and procedures.
- Ensure regulatory compliance and maintain accurate records.
- Foster teamwork among healthcare staff and communication effectively with patients and families.
- Participate in multidisciplinary meetings to address patient needs.
- Utilize electronic health records for accurate documentation.
- Provide feedback to enhance team performance.
- Stay current with nursing best practices through ongoing education.
- Mentor new staff and participate in community health outreach.
- Manage inventory and ensure availability of medical supplies.
- Perform other duties as assigned to support facility goals.
