General Description: This position is responsible for providing oversight to the delivery of support services within the Community Facilitator program; provides supervision, mentorship, and support to assigned staff (Community Facilitators) and individuals with developmental disabilities - school age and adults.
Bilingual - English/Korean; English/Armenian; English/Spanish preferred.
Essential Responsibilities/Duties:
- Ensure the provision of quality support services to individuals in accordance to the established Support Plans.
- Recruit, hire, train, and routinely monitor direct support staff.
- Required understanding of family dynamics and having a family member with a significant disability.
- Identify staff training needs and arrange for appropriate ways for the needs to be met.
- Develop individualized support plans with family members, staff, and the individual.Revise annually.
- Complete Consumer Profile. Revise as needed.
- Maintain contact and communication with assigned families.
- Ensure on-going safety requirements are implemented as appropriate and needed.
- Arrange and staff regular meetings to note progress on identified goals and plans.
- Ensure ongoing funding.
- Provide back-up hands-on support, as needed.
- Complete all documentation, as required.
- Routinely monitor and train direct support staff on proper documentation
- Complete all required paperwork and reports for assigned individuals.
- Interact with Regional Center Case Managers.
- Actively participate in the Family Support Team.
- Comply with all policies and procedures of the organization.
- Monitor support provided for quality.
- Other responsibilities as assigned.
Minimum Qualifications:
- Bachelors degree in a related Human Service field.
- Two years experience providing intensive support to individual with significant behavior challenges.
- Six (6) years hands-on experience working with persons with developmental disabilities demonstrating significant behavior challenges, may be substituted for the degree.
- Knowledge of appropriate, non-invasive behavioral supports
- Knowledge of virtual means of contact for routine use, e.g. ZOOM, HD Meetings, TEAMS, FaceTime, etc

Salary : $66,560 - $68,000
