Overview
The Quality Assurance/Quality Improvement (QA/QI) Nurse in a Home Health setting is responsible for overseeing and ensuring the highest standards of patient care by implementing, monitoring, and evaluating quality assurance and quality improvement programs. The role involves identifying areas for improvement, conducting audits, educating staff on best practices, and ensuring compliance with regulatory requirements and industry standards. The QA/QI Nurse plays a key role in enhancing patient outcomes, optimizing care delivery, and ensuring patient safety in home health services.
Duties
Quality Assurance and Compliance:
- Conduct regular audits of patient care records, documentation, and clinical procedures to ensure adherence to established standards, protocols, and regulatory requirements (e.g., CMS, state health departments).
- Monitor compliance with Medicare/Medicaid guidelines, accrediting agency standards (such as CHAP, ACHC, or JCAHO), and internal policies.
- Ensure all necessary licenses, certifications, and patient care procedures are up-to-date and compliant with regulatory agencies
Quality Improvement Initiatives:
- Develop, implement, and maintain quality improvement programs to enhance patient care, reduce errors, and improve overall outcomes.
- Identify trends in patient care data, incidents, or complaints, and develop action plans to address areas requiring improvement.
- Collaborate with the interdisciplinary team to establish and implement evidence-based best practices that drive continuous improvement in patient care.
- Use data analysis and feedback from audits to design and implement corrective actions, as well as measure the effectiveness of those actions.
Staff Education and Training:
- Provide ongoing education and training to nursing staff and other healthcare providers on quality standards, documentation, and compliance requirements.
- Lead workshops, in-services, and training sessions on the importance of patient safety, documentation accuracy, and adherence to policies.
- Act as a resource for clinical staff on quality improvement initiatives and regulatory standards.
Risk Management and Incident Reporting:
- Investigate and report any incidents or near-misses, including medication errors, falls, or adverse events, and follow up with corrective actions.
- Assist in conducting root cause analyses (RCAs) to identify underlying causes of adverse events and recommend strategies to mitigate future risks.
- Ensure that all incident reports are appropriately documented and acted upon in a timely manner.
Data Collection and Analysis:
- Collect and analyze clinical data to assess the effectiveness of patient care and identify opportunities for improvement.
- Monitor key performance indicators (KPIs) such as patient satisfaction, readmission rates, patient outcomes, and staff performance.
- Compile and present reports on QA/QI activities, audit results, and improvement efforts to management and leadership.
Collaboration with Leadership and Interdisciplinary Team:
- Work closely with the Director of Nursing, clinical managers, and other team members to ensure quality standards are being met across the organization.
- Participate in regular meetings with the leadership team to review quality metrics and performance data and make recommendations for improvements.
- Collaborate with other healthcare providers (e.g., physicians, therapists, social workers) to ensure holistic, patient-centered care
Patient and Family Advocacy:
- Ensure that patient feedback is considered when evaluating the quality of care and addressing areas for improvement.
- Address patient and family concerns or complaints related to the quality of home health services, resolving issues in a timely and professional manner.
Regulatory Reporting:
- Ensure timely reporting of any regulatory violations or incidents as required by state, federal, and accreditation agencies.
- Assist in preparing for and participating in state/federal surveys or accreditation visits.
Qualifications:
Education:
- Registered Nurse (RN) with an active, unrestricted license.
- Bachelor’s degree in Nursing (BSN) preferred; Master’s degree in Nursing, Healthcare Administration, or a related field is a plus.
Experience:
- Minimum of 3-5 years of nursing experience in home health care, with at least 1-2 years in a quality assurance, quality improvement, or supervisory role.
- Experience with home health care regulations, policies, and compliance standards (e.g., Medicare, state health regulations) is required.
Skills:
- In-depth knowledge of quality assurance and quality improvement methodologies, including root cause analysis, corrective action planning, and process improvement tools (such as PDSA or Six Sigma).
- Strong analytical and data interpretation skills, with experience using software or systems for quality monitoring and reporting.
- Excellent organizational, communication, and interpersonal skills.
- Ability to work independently and as part of a team to drive improvement initiatives.
- Proficiency in the use of electronic health records (EHR) and other healthcare management systems.
Certifications:
- Certification in Quality Improvement or Quality Assurance (e.g., CPHQ – Certified Professional in Healthcare Quality) is preferred.
- CPR and BLS certification as required by the organization.
Work Environment:
- The QA/QI Nurse typically works in an office or administrative setting, with occasional travel to patients' homes to observe care delivery or conduct audits.
- Some evening or weekend hours may be required to meet deadlines or address urgent issues.
Physical Requirements:
- Ability to lift and carry up to 25 pounds, as needed.
- Ability to sit for extended periods and use a computer and other office equipment.
