Younity Access Center is seeking TWO Case Managers for our Health Transitions Program who are passionate about improving the lives of youth and young adults through positive peer connections and community-based support. We are looking for someone who is passionate about empowering young adults who may be falling through the cracks of traditional supports and services. Younity Access Centers are innovative programs serving young adults ages 16-25 who experience or are at risk of developing serious mental health concerns and/or substance use. The program is designed to be flexible, offer low/no barrier access to services and supports, and to rapidly respond and link young people to needed resources. The Transition to Independence (TIP) Facilitator/Case Manager will work with the Program Director and their colleagues to ensure participants receive culturally relevant, inclusive, accessible services.   

IN THIS ROLE YOU WILL:

  • YOU WILL provide coaching and facilitation to transitional-age youth in meeting their identified life goals.  
  • YOU WILL USE a strengths-based approach to facilitate the development of life skills.
  • YOUR WORK WILL maximize the likelihood of success for each youth served.  
  • YOU WILL assist youth in developing their confidence and independence in support of their transition into adulthood. 
  • YOU WILL provide case management services within the context of the youth’s culture following the TIP model guidelines. 
  • YOU WILL support youth in achieving their goals pertaining to housing, education, career, stabilization, mental and physical health, and the creation of natural supports. 

WHAT WE ARE LOOKING FOR IN A CANDIDATE:

  • Education: A bachelor's degree in the social sciences, rehabilitation or education field preferred. Experience may substitute for education. High school diploma or HiSET required. 
  • Experience: At least 2 years' experience working in educational, social service, juvenile justice, and/or mental health settings providing counseling, training, therapy and/or guidance to young adults and their families. 
  • Valid MA Driver’s License and regular, reliable access to a MA-registered/insured vehicle. 
  • Lived experience utilizing housing, financial, substance-use, and/or mental health resources preferred. 
  • Experience working with specialized populations of adolescents preferred, such as those involved with DCF, DYS, those experiencing homelessness, and youth who identify as LGBTQIA2S . 
  • Familiarity with Motivational Interviewing, the TIP Model and peer-support approach preferred. 
  • Ability to demonstrate a commitment to social justice, equity, & inclusion, and be able to share examples of how they have operated from this perspective. 
  • Comfort and familiarity with engaging in sensitive conversations such as those pertaining to homelessness, substance use, incarceration, self-harming behaviors and suicidal ideation. 
  • Acceptable CORI (Criminal Offender Record Information), OIG background check required.
