Join our team of compassionate Social Workers dedicated to empowering patients and their families to achieve optimal psychological, emotional, spiritual, social, and physical well-being. Our Social Workers identify and foster coping skills and strengths within patients and families. They play a crucial role within the interdisciplinary team, addressing psychosocial stressors and collaborating to enhance the quality of end-of-life care for patients and their loved ones.

RESPOSNSIBILIES:

  • Conduct initial psychosocial evaluations within required time limits, following hospice regulations and Conditions of Participation.
  • Provide guidance on advanced care planning, including directives, DNRs, DPOAs, and funeral planning, as per patient and family wishes.
  • Assist patients and families in understanding and following medical recommendations for optimal social and health adjustment.
  • Utilize available resources to support patients and families living with illness limitations.
  • Effectively manage daily tasks, balancing priorities and completing duties independently.


SCHEDULE:

The schedule for this position entails Monday through Friday day shifts. On-call duties, which may include after-hours and weekend work, will be rotated based on location.

Requirements:

QUALIFICATIONS AND LICENSURE:

  • Graduate of an accredited school in social work, psychology, or other related field.
  • Licensed Social Worker licensure with no encumbrance, preferred
  • Compassionate and empathetic nature, with a genuine desire to provide comfort and support to patients and families/loved ones during a challenging time.
  • Valid driver’s license with current automobile insurance
  • Reliable vehicle
