Current Opening for a Care Manager in Passaic County.

This position would also require the candidate to occasionally work after hours, covering our on call service which runs Monday-Friday 5pm-8:30am and Saturday and Sunday, each shift broken down into 4 hours each. Is that something your agency allows workers to do?

The New Jersey Office of the Public Guardian for the Elderly (OPG) acts as a surrogate decision-maker for residents sixty (60) years of age and over who have been deemed incapacitated by the Superior Court of New Jersey.

OPG is seeking a field-based care manager to take on the following responsibilities:

Manages a caseload of OPG wards. 

· Assists the Public Guardian in managing OPG client health and well-being by checking on clients through periodic visitations; preparing prearrangements for funerals; communicating with family members and medical staff about client health and general well-being; approving basic medical procedures; managing living arrangements; evaluating, monitoring, and approving healthcare options and treatment plans; and ensuring home clients receive appropriate services such as grocery shopping, transportation, aids, and other home assistance. 

· Keeps and updates detailed progress notes based on all interactions and services provided.

·  Completes psychosocial assessments to obtain information on the mental status, physical status, social history, and current living arrangements for elderly individuals. Presents case findings, challenges, and recommendations at weekly multidisciplinary team meetings. 

· Monitors clients for signs of abuse, neglect, or exploitation from facilities, friends and family, and/or personal neglect. 

· Advocates for client preferences, rights, and freedoms to all invested parties such as medical staff, OPG staff, healthcare facilities, and family members

· Works with Adult Protective Services to protect OPG wards as the legal decision-maker by intervening and/or removing OPG clients from abusive, neglectful, or exploitative situations.

· Rotates on-call coverage with other care management staff to be available for client medical emergencies. Makes procedurally-appropriate decisions on behalf of the client or escalates calls to the supervisor or attorney on-call.
