What are the responsibilities and job description for the Health Navigator position at La Colaborativa?
As a public facing member of La Colaborativa, the Health Navigation Specialist is tasked with providing navigational support, guidance, and culturally congruent information to advance health equity for the Latinx and immigrant community, regardless of household income and immigration status. Through a culturally congruent, human-centered, approach, the Health Navigation Specialist will serve community members systemically denied access to culturally accessible, affordable physical and mental healthcare. A pivotal role functioning independently and as part of a team, the Health Navigation Specialist will gather information on healthcare needs; identify prospective resources and key barriers that may inhibit access; determine eligibility for care; assists with various application processes; facilitate enrollment in MassHealth, HealthConnector, and Health Safety Net insurance services; and performs tactful, effective advocacy on behalf of community member, while providing navigational services to ensure residents can overcome barriers to culturally appropriate, affordable care. Additionally, the Health Navigation Specialist is responsible for crafting and delivering bilingual health information, education, and outreach across a variety of accessible, community-based locations
Key Responsibilities
- Perform human-centered intakes, collect and analyze data, perform triage, and conduct needs assessments to determine short and long term physical and mental health needs;
- Develop and implement trauma informed, culturally sensitive healthcare and health service delivery plans;
- Facilitate enrollment, support redeterminations and appeals, troubleshoot issues, and provide navigational services across various public healthcare programs, including, but not limited to, MassHealth, Health Safety Net, and Health Connector;
- Assist members with identifying, accessing, and navigating affordable primary care, preventative care, healthcare, integrative mental health, and specialized community-based services;
- Arrange wraparound services, such as transportation and economic assistance, by collaborating with interdisciplinary teams on service coordination cases;
- Exercising empathy, develop strategies to assist members in overcoming barriers to health and wellness in an empowering, sensitive manner, recognizing the history of disenfranchisement of BIPOC communities by the healthcare system;
- Empathetically mediate between members and health and human service agencies, troubleshoot unforeseen issues, and advocate tactfully on behalf of members;
- Assist with the management of health equity initiatives designed to address health disparities;
- Craft and implement bilingual, culturally appropriate health literacy and health education campaigns on a variety of focal areas. Host at least 16 “office hour” events per month for health insurance enrollment, complemented by resource education workshops, neighborhood canvassing, and informative sessions on a variety of healthcare issues of interest to the community;
- Consult with members on healthcare costs, financial assistance resources, and medical debt mitigation strategies;
- Facilitate meaningful referrals to specialized clinical and non-clinical wraparound services. Work closely with partner agencies to deliver impactful and coordinated services;
- Forge collaborative relationships across Departments. Foster and maintain robust partnerships with community based and healthcare organizations;
- Advise leadership on emerging trends, chronic barriers, and systemic issues with implications for the public health and welfare of community members;
- Maintain an accurate understanding of the changing landscape of healthcare and mental healthcare;
- Comply with all MassHealth CAC regulations, continuing education requirements, and program policies;
- Perform data collection, prepare regular reports, and routinely update leadership in a timely manner;
- Prioritize cultural safety and sensitivity, specifically with Latinx and immigrant communities, in all facets of work; and
- Other duties, as assigned.
Qualifications and Skills
- Bilingual Spanish-English or Haitian Creole-English;
- Bachelors Degree preferred;
- 2 years of resource navigation, client advocacy, case management, or customer service experience, preferably in human services possess demonstrated commitment to customer service;
- Must be over 21 years old;
- Knowledge of MassHealth and HealthConnector, as well as public and private healthcare services;
- Experience working harmoniously in immigrant and multi-cultural communities;
- Demonstrable track record of working independently and dependably completing assignments;
- Highly skilled at written and oral communication, as well as tactfully interacting with people through a trauma-informed, culturally sensitive approach;
- Excellent ability to carry out responsibilities independently as well as collaboratively as a team;
- Capable of being adaptable, agile, and proactive;
- Detail oriented, methodical, and organized, with excellent time management and organizational skills;
- Experience working in an innovative manner, tackling challenges and assignments in unconventional ways or on tasks that require creativity;
- Background in excelling in a fast-paced, dynamic environment. Experienced at creating and implementing systems to remain organized and complete tasks on-time; and
- Empathetic and compassionate, with a strong sense of community.
