What are the responsibilities and job description for the Family Coach position at Lakes & Prairies Community Action Partnership?
JOB FUNCTION:
Provide intentional, intensive, family-centered coaching to families that are working toward economic security. This position meets people where they are at and partners to empower families to achieve their hopes and dreams. Programming is trauma-informed and carried out through a racial equity lens. Services are relationship based, flexible and adaptable. Collaborate with all Divisions and communicate with program community partners. Provide oversight, community outreach, and direct support services to eligible families and individuals through Economic Empowerment Programs designed to help families achieve economic success including Homeownership Programs.
QUALIFICATIONS:
Minimum
- Associate or Bachelor’s Degree in a Human Service Related Field
- Must hold a valid driver’s license and current vehicle insurance
Preferred
- Experience in Community Action
- Lived experience or a history working with individuals with low incomes

HEAD START PROGRAM REQUIREMENTS:
- Physical exam within 30 days, 3-year renewal
- First Aid, CPR certification
- Hepatitis B or documentation of refusal
- CARS training, 5-year renewal
Whole Family Services:
- Coordinate with agency team members to identify, select, and enroll families, in a timely manner, who are ready to work toward achieving their hopes and dreams of lifting their family out of poverty
- Identify and recruit Whole Family households
- Working with families collaboratively to identify hopes and dreams through goal setting
- Provide multi-generational, intensive services and supports that empower families to achieve their hopes and dreams
- Track outcomes for both children and the adults in their lives and complete required program reports as requested
- Engaging family voice in the planning and implementing of the Whole Family services by responding to feedback
- Provide high quality services that are evidence and science based including utilizing Family Centered Coaching skills and tools
- Complete and enter client data including documentation of demographics, services, and outcomes into data collection systems
- Initiate and coordinate with CAPLP team members and other service providers to provide a holistic set of services to help the family achieve economic self-sufficiency
- Analyze program data to adjust and improve services
- Collaborate with Head Start staff to learn and support PFCE (Parent-Family-Community Engagement) systems
- Approach working with families in a positive, welcoming, and energetic manner Provide families flexible scheduling based on their needs

Homeownership Programs
- Promote awareness of these programs within the community and build community partnerships
- Identify, recruit, and encourage potential participants
- Accurately complete intake packets and verify information to determine eligibility for these programs
- Complete and enter client data into data collection system
- Provide case management services to assigned participants
- Provide information and referrals when necessary and act as a liaison and client advocate for referrals as appropriate
- Maintain accurate and detailed client records according to program requirements
- Work cooperatively with program staff to provide high quality Homebuyer Education workshops
- Assist with program reporting as requested
Family Assets for Independence in MN (FAIM):
- Coordination of the FAIM program including promotion, enrollment, case management and reporting
- Maintain partnerships with referring agencies for both programs
Agency and Division Support:
- Represent Agency in the communities we serve
- Compile and submit required reports and data to Supervisor
- Assist with other program projects as requested by Supervisor
- Attend and participate in agency, community, and state meetings important to effective Agency operations
- Assess needs for professional development and conduct training for intra/interagency staff as required
- Availability to work designated Agency hours for your Division and position
