WHO YOU CAN BECOME: The Case Manager (RCM) is a full-time, 40 hour a week, non-exempt, and hourly position who is responsible for rapid engagement and ongoing assessment and support of the needs of the family of youth enrolled in the Juvenile Reporting and Family Center program. They will work with the JRFC team and community resources to foster youth and family safety, well-being and permanency within the home through family focused team meetings, individual contacts and referrals. The JRFC Caseworker will work as a member of the JRFC team across sites. They will be an integral member of the Centralized Intake Team, with scheduled on call responsibilities to respond to intake needs and coverage for same day admission to the program at any of the sites. The JRFC Caseworker will develop an understanding of the most effective interventions to support alternative to detention services with youth involved in the juvenile justice system.

WHAT YOU’LL DO:

  • Communicate and work as a member of the team with the Reporting Center staff, Probation and the Department of Children, Youth, and Families (DCYF).
  • Meet weekly with mentors to gather information about youth’s individual activity plans.
  • Maintain accurate online calendar of meetings/appointments with youth, family and providers (calendar should be completed on a weekly basis).
  • Responsive to the family’s needs, linking them to community and natural resources to increase protective factors.
  • Represent the program at court appearances, Probation, DCYF, medical, educational and/or counseling appointments as needed/required.
  • Conduct mental health and substance abuse screenings utilizing recognized and approved screening instruments.
  • Maintain CANS NY certification
  • Complete CANS NY assessments for each youth.
  • Prepare reports of findings to include recommendations and referrals when appropriate.
  • Participate in regularly scheduled Intake Team Meetings and Reporting Center team meetings.
  • Plan and facilitate Family Team Meetings, home visits and family and youth contacts as per contractual requirements. Per county contract requires a minimum of one (1) hour of contact with each family.
  • Provide educational support through school visits, conferences and as a liaison between the youth’s school and the Reporting Center to support the youth’s academic progress, attendance and attachment to his school.
  • Professional completion of all casework documentation, including assessments, recommendations, referrals, FTMs, discharge summaries, case files, Awards and Connections progress notes, and Family Assessment and Service Plans within established Office of Children and Family Services and county expectations and timelines.
  • Ongoing outreach with community agencies and resources for family support.
  • Attend evening Parent Nights; assist in facilitating a Parent Advisory Group.
  • Facilitate casework transportation needs for youth and families, as needed.
  • Assist in tracking and achievement of outcomes.
  • Provide information to the records department on Intakes and Discharges for La Salle JRFC youth.

WHAT YOU NEED:

  • Bachelor’s Degree with relevant experience.
  • New York State Driver’s License in good standing and vehicle.

WHAT WE OFFER:

  • For full-time employees: Start at 10 days off for vacation, 13 paid holidays a year, 3 personal days, 12 sick days per year and other leave time available dependent on the circumstances.
