WHO YOU CAN BECOME: The Residential Case Manager (RCM) is a full-time, 40 hour a week, non-exempt, and hourly position who provides supportive and therapeutic services to youth and families within LSS’ residential, after care and community-based programming. They will support all agency efforts to assist youth/families to successfully navigate foster care, juvenile justice, educational, and other systems. The RCM partners with assigned clinical and agency staff to advance a youth and family’s treatment and permanency goals. They assist with all relevant Office of Children and Family Services, court, and other required documentation and act as a liaison between the treatment team and all family and community partners (e.g., family court, Department of Social Services (DSS), Probation, School District, etc.).
WHAT YOU’LL DO:
- Provide case management support to all assigned youth.
- Arrange, support, and supervise family visits at LSS campus, as well as in community.
- Provide transportation and other assistance to youth and families to attend treatment and other related appointments (e.g., family court, medical, home visits, probation, etc.).
- Complete all assigned case documentation and record maintenance (progress notes, Family Assessment and Service Plans, Casey Life Skills, Credit Checks, etc.), as required by all relevant Agency, contractual, and regulatory expectations.
- Actively participate in treatment team discussions and planning and treatment delivery.
- Communicate critical case related information in a timely manner to all team members and external stakeholders (DSS, parents, family court, local school district, etc.).
- Support youth and family progress toward safety, permanency, and successful treatment outcomes.
- Acts as third-party reviewer for cases not on assigned caseload.
WHAT YOU NEED:
- Bachelor’s Degree in Social Work, Psychology, or related human service field.
- New York State Driver’s License in good standing as determined by the Agency.
WHO ARE YOU:
- Able to engage and support youth/families to be successful in treatment.
- Self-directed and able to navigate both internal and collateral multi-disciplinary teams.
- Can communicate effectively verbally and in writing about complex treatment goals with youth, families, treatment team members and external stakeholders.
- Able to be a professional representative of the agency and be capable of assisting the youth and family navigate our agency as well as placing agencies.
- Comfortable with testifying on behalf of the agency/treatment team in Family Court.
- Can work collaboratively with DSS and other referral/discharge resources to ensure safety, progress, and permanency for youth.
- Able to work on campus, in family home, family court, probation and other community- based settings
