Title: Family Support Provider
FLSA Status: Non-Exempt
Reports To: Project Director
Position Summary
A Family Support Provider (FSP) works with a Care Coordinator to provide targeted support to address the needs of the child and family being served. They work directly in the home and community of the family. The FSP helps identify the family needs and focuses on the child’s developmental age and cultural heritage. This is the Wraparound process under the general supervision of the Project Director and/or Assistant Project Director. The FSP utilizes best practices as communicated by the State of Oklahoma and the LCDA policy.

Essential Job Duties
· Support for the child and his/her family.
· Creating a family plan and course of action based on the individual needs of a family.
· Coordinating between all program resources and benefits identified.
· Educating the family concerning agency interactions, benefits, and programs.
· Evaluating situations using discretion and personal judgment to adopt an effective course of action, to delegate responsibilities, organize facts, and present them effectively.
· Providing consultation and expert advice to management regarding family plans and advocacy.
· Using discretion and independent judgment to interpret and implement policies regarding family plans and advocacy.
· Using personal judgment, identify available options for agencies and organizations as appropriate to meet a particular family’s needs.
· Advocate for the family in the agency setting, and with other agencies and organizations identified.
· Collaborate with families and professionals.
· Fill out and submit forms as required.
· To attend or speak at conferences and or meetings.
· Other duties as assigned.

· Exhibit ability to effectively organize and plan field-work schedule to best service and maximize the needs of consumers, i.e. arranging home-based meetings, agency meetings, and care coordinator meetings.
Supervisory Responsibility
This is not a supervisory role.
Work Environment
This job operates in a clerical, office setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Position Type and Expected Hours of Work
This is a full-time position with typical working hours of Monday through Friday, 8:00am to 5:00pm. Hours and work shifts may change in accordance with business needs. Exempt employees must have the ability to be on-call and available, as business needs require, Evenings, Saturday or Sundays if necessary.
Required Minimum Job Qualifications, Experience, Education and Training
Education: High school diploma or equivalent
Experience: Must have experience working with a child who has emotional support needs.
Skills: Specialized training and/or certification in mental health services or related field as required in order to meet requirements for billable services and/or adjunctive services, i.e. specialized certifications (BHRS) Case Management, SOC training and updates. (These certifications are usually acquired through the LCDA agency once hired)
License: Current Operators Driver’s License
Additional Eligibility Qualifications
Must be able to accomplish all duties with little to no assistance.
