Laurel Parc, a beautiful senior living community just north of the Beaverton-Hillsboro area seeks a Licensed Practical Nurse to join our care team! This position will administer nursing care and perform site visits for residents within our Assisted Living & Memory Care departments under supervision to ensure the ultimate level of resident health, comfort and safety. The LPN will also help ensure staff are trained on proper medical procedures and following current regulations and standards. Most importantly, the LPN will relate professionally with residents, staff and family members, display a positive attitude and work as a team!
Schedule: Monday-Friday 8:30am-5:00pm (may be able to provide some flexibility with schedule).
Pay: $42 - $43 per hour
Status: Full-time
Main Responsibilities:
- Monitor, document and report changes of condition and participate in creation & updates of care plans.
- Evaluate residents referred to community and determine best services provided & proper placement.
- Obtain initial and ongoing referral documentation from physicians, insurers, and providers.
- Ensure physicians orders are followed; including appointments, and coordinate care as needed.
- Properly communicate resident & family concerns with staff.
- Oversee medications, medication renewals, medication sheets & necessary follow-up
- Initiate care with service providers and ensure staff have proper equipment & supplies.
- Assist with answering call lights, serving meals, bathing, and dressing as needed.
- Ensure implementation of proper policies, procedures, compliance & standards of care.
- Utilize health record management software.
- Be available for on-call medical questions and/or emergencies via phone, per on-call schedule.
Qualifications:
- High school diploma or equivalent and current certification as LPN
- One year experience in home health, assisted living or long-term care preferred.
- Documentation showing full vaccination against COVID-19
- Professional verbal and written communication

Language Requirement
All team members must have the basic ability to use computer/smart phone applications for training and company system purposes. Employees must also be able to comprehend written documents such as work orders, safety rules, instructions, policies, and procedures. Also required is the ability of employees to speak and communicate effectively with clients, coworkers and management.
