Liberty Village of Danville is a beautiful retirement community that is looking for an enthusiastic and motivated Individual to lead our social services department in our state of the art long term care facility.
Director of Social Servcies Responsibilities:.
- Complete tours
- Complete admission paperwork.
- Serve as a liaison between facility, residents, responsible parties, and outside agencies
- Facilitate the discharge planning process; development and implementation of discharge care plans.
- Refer resident to outside agencies as appropriate.
- Provide information to resident/responsible parties as to Medicare/Medicaid,and other financial assistance programs available to the resident in conjunction with the Business Office Manager.
- Keep abreast of current state and federal regulations, professional standards, and facility policy and procedures.
- Attend meetings as required.
- Complete observations and additional documentation as required

Job Type: Full-time
Pay: $17.00 - $19.00 per hour
Expected hours: 30 per week
