Lifeway Services LLC, is a Residential Care Agency for the intellectually disabled and adults with autism. We are actively seeking a responsible and dependable LPN for a Site Manager position.
Site Manager Responsibilities Include but not limited to:
Oversight of Direct Support Professionals and Program operations
Health and Safety of Individuals served
Implementation of ISP Services
Monthly collection and review of residential service documentation
Communication to Program Specialist regarding all areas associated with the implementation of contracted services
Ensure appropriate staffing as indicated in each Individuals ISP

Requirements:
LPN
Age 18 or older
Open Availability
Must have valid Drivers License and vehicle registration
Proof of auto insurance
