What are the responsibilities and job description for the RN Care Manager position at Mackinac Straits Health System?
Mackinac Straits Health System is looking for a full-time RN Care Manager to join our clinic! This position would support our Mackinaw City and Cheboygan clinics.
Essential Duties include (but not limited to):
· Collaborates with physician and staff to identify and manage the care of the patients.
· Co Formulates and implements a care management plan that fits patient/ family needs
· Conducts initial and periodic assessments for care management population.
Evaluates the effectiveness of care plans and modifies as necessary.
· Promotes self management for the patients and empowers patient achievement of maximum wellness and independence.
· Serves as a liaison for providers, patients and families.
· Develops collaborative working relationships with insurance case managers and negotiates with third parties for cost effective, high quality services.
· Demonstrates understanding of regulatory requirements and collaborates with staff to meet such requirements.
· Accessible during after hours and responds to on call issues promptly and appropriately.
· Other duties as assigned
Qualifications Required:
- RN licensure required, BSN strongly preferred
- 2-3 years of clinical leadership experience
- May work in other clinics in MSHS System
- Customer service focused
- Able to develop positive relations with staff
- Positively represents the practices both within and outside the organization
- Exemplary attendance record
- Computer literacy with Word, Excel and EMR (All Scripts)
- Able to work a flexible schedule and coordinate multiple projects simultaneously when needed
