Mainstay Supportive Housing & Home Care, Inc. is Hiring a Resident Service Coordinator Near East Brookfield, MA

Mainstay Supportive Housing & Home Care, Inc. is currently seeking a part-time dedicated RESIDENT SERVICE COORDINATOR to join our growing workforce in Spencer, MA. The Resident Service Coordinator assumes responsibility for service management function within assigned facilities. This function includes development of contacts with service providers and agencies for resident referrals and the ongoing service management responsibility. Educates residents on available services and monitors provisions of services. The Resident Service Coordinator works in conjunction with the Resident Manager(s) and other management staff of the facility.

  • Assists and advises residents and families of the services available which may help resident to maintain a self-reliant lifestyle.
  • Promotes wellness activities for all residents.
  • Educates residents, families, and staff on available community resources.
  • Assists residents in building informal support networks among themselves and with family members.
  • Acts as a liaison between community agencies, service providers, and residents.
  • Follows up on resident situations the resident manager(s) believe may need supportive services
  • Encourages residents to be proactive in meeting their social, psychological, and physical needs.
  • May assist residents or coordinate training for residents in understanding lease and tenancy obligations.
  • Monitors the delivery of services to residents to ensure they are appropriate, timely, and satisfactory.
  • Provides limited case management for a resident when such service is not being provided by the general service community.
  • Educates residents on service availability, application procedures, resident’s rights, etc. both individually and as a group.
  • Submits biannual report to HUD.
  • Sets up volunteer support programs with service organizations in the community.
  • Advocates and may negotiate on behalf of residents for adequate provision of services
  • Meets with service providers as needed and appropriate.
  • Assembles and keeps current a directory of community services and makes it available to residents, families, and management.
  • Assists management in identifying residents who need assistance. Educational programming that empowers and enhances resident independence is appropriate.
  • Maintains emergency contact information for each resident.

Education Requirements:

High School Diploma, College Degree - Concentration in Social Work, Gerontology, Psychology or Public Health preferred.

Relevant Work Experience:

  • Positive client demeanor
  • Ability to build rapport with staff teams, clients, and family members.
  • Excellent written and verbal communication skills
  • Ability to work independently with minimal supervision
  • Excellent organizational skills
