As a Health Coach at Maple Manor, you will have the opportunity to work with patients who are seeking to improve their health and well-being through personalized coaching and support. You will play a crucial role in empowering our patients to make positive lifestyle changes and reach their health goals.
Key Responsibilities:
- Conduct initial assessments to determine patients' current health status, goals, and barriers to achieving those goals
- Develop personalized health plans and strategies based on patients' needs and goals
- Provide ongoing support and guidance to patients through regular coaching sessions
- Develop and implement health and wellness programs and workshops for patients and staff
- Collaborate with the interdisciplinary team to ensure that patients' health plans align with their medical treatment plans
- Monitor and track patients' progress towards their health goals and make adjustments to their plans as needed
- Educate patients on healthy habits, nutrition, exercise, stress management, and other health-related topics
- Serve as a role model for healthy living and promote a positive and supportive environment for patients and staff
- Stay current on industry trends and research related to health coaching and share knowledge with colleagues and patients
Qualifications:
- Bachelor's or Master's degree in Health Promotion, Public Health, Nutrition, Exercise Science, or related field
- Certification as a Health Coach is preferred, but not required
- Minimum of 1-2 years of experience in health coaching or related field
- Excellent communication, interpersonal, and motivational skills
- Strong knowledge of nutrition, exercise, stress management, and behavior change theories and strategies
- Ability to work independently and collaboratively with a diverse patient population
- Empathetic and non-judgmental approach to coaching and counseling
- Experience working in a healthcare or rehabilitation setting is a plus
