Shift is 3p-11p with $1.00 an hour shift differential pay. Schedule is Monday - Friday

The Licensed Staff Nurse communicates with off going and arriving shifts of personnel to assure the continuum of quality of care. The Licensed Staff Nurse makes rounds, evaluates the physical, emotional and social needs of the residents to ensure the staff assigned provides intervention as indicated. Supervises the care provided by the Nursing Assistants assigned to the unit, providing intervention, and education as indicated. Prepares and administers medications and treatments according to procedures. Consults with physicians, receives, records and transcribes orders as required. Reviews resident’s care plans and assures these plans are followed. Documents on records as per policy and procedures of the facility.

Qualifications

1. Demonstrates leadership and managerial ability.

2. Has the ability to organize, plan, and supervise the Nursing Assistants assigned to work a designated unit and shift.

3. Maintains effective interpersonal relationships and works well with the general public.

4. Is of good emotional, mental and physical health, having sound judgment and high professional standards.

5. Must be willing and capable of providing emergency care as needed for any resident in the facility.

6. Is courteous in working with residents, families and employees.

7. Maintains a neat, well-groomed and professional appearance at all times.

8. Must be willing to participate in the on call schedule

9. Must be willing to work preferred shifts as scheduled and assist on other shifts as needed to assure adequate staffing to meet the needs of each resident.

Responsibilities

The Licensed Staff works with the Director of Nursing to assure the following responsibilities are accomplished.

1. Receives report from the off going shift and gives report to on coming shift.

2. Assigns nursing assistants and other designated personnel assuring each resident’s needs will be met during the assigned shift.

3. Gives report to Nursing Assistants.

4. Makes rounds of the assigned unit frequently during the shift providing intervention as needed. During the entire shift, will be attune to needs of the unit while doing other nursing tasks.

5. Prepares and administers medications as ordered using the 5 rights method. Reorders medications as needed in preparation for future shifts. Assesses and documents medication issues as per policy.

6. Completes treatments as ordered. Assesses and documents accordingly. Contacts the physician for any treatment issues as per policy.

7. Monitors residents for risk or complications as a result of the residents’ medical conditions. (i.e., skin breakdown, incontinence, contractures, reduced mobility, reduced involvement in bathing, dressing, or eating, weight loss, behaviors due to change in lifestyle, etc.)

8. Observes and monitors the Nursing assistants to assure that resident’s needs are met and nurse aides duties are performed.

9. Document on nurse’s notes as per policy. Assures there is accurate and timely documentation of any unusual observation, incident, or situation.

10. Reviews and follows resident’s plan of care.

11. Assists in the ordering and reviewing of results of laboratory tests as indicated. Notifies physician of results.

12. Counts required medications with oncoming and off going shifts.

13. Keeps nurse’s station, medication room and other work spaces clean and orderly.

14. Participates with month end physician order reviews

15. Makes rounds toward end of shift to assure area is in order for the oncoming shift.

16. Reviews and assures there is adequate staffing for the oncoming shift. Replaces known absences in a timely manner.

17. Notifies the Resident and/or Resident Representative of changes in condition or changes to the plan of care

18. Prepares incident/accident reports as needed
