POSITION TITLE: Enhanced Case Manager Team Leader
SALARY: Medical, dental, vision, life/AD&D, Aflac Supplemental, Flexible Spending Account, CalSavers Retirement Savings Program. Paid time off, sick days, and paid holidays. Mileage Reimbursement.
SALARY RANGE: $50,00-55,000, Non-exempt, Full Time
REPORTS TO: ECM Program Manager
Work Schedule: MHASD operates on a 40 hour work week; typical schedule is 8:30 a.m. to 5:00 p.m. Monday through Friday. However, flexibility is required due to the nature of business. Schedule may be changed as needed to include evenings, weekends and holidays.
Program Overview: Enhanced Case Manager program (ECM) provides individuals, youth, pregnant women, individuals returning from incarceration, tailored support services which address their needs and provide assistance in ensuring health/behavioral health services are provided to Medi-Cal eligible individual needing care. Team Leader will support all activities of ECM program.
ECM Team Leader Responsibilities:
- ECM Team Leader will work closely with Program Manager and ECM Staff.
- Provides ECM services and support assigned ECM Workers in providing care.
- Support ECM Program Manager in assigning cases and supporting staff in completing program services.
- Provide program support in all aspects of ECM services and operational support in a culturally sensitive and appropriate manner.
- Responds to inquiries related to ECM and supports ECM staff.
- Assist with data entry, reports, and weekly case assignments.
- Organizes and participates in operational meetings.
- Collaborates with other community providers to ensure adequacy of community resources.
- Responsible for maintaining inventory of ECM materials.
Qualifications:
- Bachelor’s degree in public health or social work required. MSW or MPH preferred.
- Ability to work independently with limited supervision.
- Reliability and demonstrated good judgment under stress.
- Ability to multi-task and remain organized in a fast paced environment while maintaining emphasis
on details and planning.
· Strong customer service skills, and problem solving skills
· Computer skills including: Microsoft Office, Excel, Outlook, PowerPoint, Etc.
· Must pass Background Clearance.
· Must have current CA Driver License and current car insurance.
