Join our dedicated team as a Licensed Practical Nurse (LPN) in our New Way Services (NWS) Division, where we provide critical care and support to individuals living with acquired brain injuries (ABI) or disabling medical conditions, such as multiple sclerosis or severe diabetes. As an LPN, you'll play a key role in supporting residents across 3-4 residential group homes. This role is ideal for nurses seeking meaningful work, a positive work-life balance, and a supportive environment.
What You’ll Do:
- Provide triage and nursing support for residents, ensuring timely responses to medical needs.
- Conduct regular visits to group homes for health assessments, staff support, and participant education.
- Monitor and manage health conditions, such as diabetes, while coordinating care plans.
- Complete medication audits and ensure compliance with MAP policies.
- Accompany or arrange participant transport for medical appointments.
