What are the responsibilities and job description for the Children's In-Home Case Manager position at Mental Health Center of North Central Alabama?
- Intensive in‐home intervention services.
- Resource Needs Assessment: (Examples of tasks: using required forms; determines through assessment clients assets, deficits and basic needs; review needs on regular basis.)
- Facilitates Treatment Plan development and implementation in coordination with Master’s Level Team Member. (Examples of tasks: coordinate care needs that may include basic living needs, medical services, financial aid, etc.)
- Implements plan by providing case management services and functions as co‐therapist. (Examples of tasks: monitor client condition; make home visits; assist client in securing needed services; assist Master’s Level Team Member in behavior plans, family/parent training and support, crisis intervention, etc.)
- Represents the needs of the seriously emotionally disturbed. (Examples of tasks: establish liaison with other community resources; identify service gaps; advocate for clients.)
- Participate in insuring quality of services. (Examples of tasks: participate in case management training; complete work in accordance with standards; participate in staff meetings; participate in peer review; participate in in‐Home training and updates.)
- Basis Living Skills Instructor. (Examples of tasks: provides didactic, process, and experiential instruction to clients.)

Pay: $14.44 - $15.87 per hour
