Description
The Intensive Case Manager is responsible for providing comprehensive case management services to high school students facing significant barriers to academic success. This role involves identifying students in need, developing individualized support plans, coordinating services, and collaborating with school staff, families, and community resources to ensure students achieve their educational and personal goals.
Case Management:
- Identify and assess high school students who require intensive support.
- Develop and implement individualized case plans that address academic, social, emotional, and behavioral needs.
- Monitor student progress and adjust plans as needed to ensure ongoing support and success.
Collaboration and Coordination:
- Work closely with school counselors, mental health teams, teachers, administrators, and other staff to identify and support at-risk students.
- Coordinate with external agencies, community organizations, and service providers to connect students and families with additional resources.
- Participate in the student success team to discuss student progress and coordinate interventions.
Student and Family Support:
- Provide direct support to students, helping them navigate challenges and develop coping strategies.
- Engage and communicate effectively with families to involve them in the student's support plan and address any barriers to their involvement.
- Conduct home visits as necessary to better understand and support the student's environment and needs.
- Attend court hearings, medical appointments, and other relevant meetings.
- Complete referrals and monitor participation in mental health and after-school programming.
Documentation and Reporting:
- Maintain accurate and up-to-date records of all case management activities and student progress.
- Prepare reports for school administration, detailing the outcomes of interventions and suggesting improvements.
- Ensure compliance with all school policies, legal requirements, and ethical standards in the documentation and handling of student information.
Training and Professional Development:
- Participate in ongoing professional development to stay current with best practices in case management and student support.
Qualifications
Education:
- Bachelor’s degree in social work, counseling, psychology, education, or a related field. A Master’s degree is preferred.
Experience:
- Minimum of 2-3 years of experience in case management, counseling, or a related field, preferably in an educational setting.
