What are the responsibilities and job description for the Home Health Nurse position at Morrill County Community Hospital?
The Home Health Nurse provides nursing care to patients in their homes, offering a range of medical services to ensure their comfort, safety, and recovery. They work closely with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Contain costs through the wise use of time, mileage and materials.
- Provide case management as described in state regulations for all cases involving nurse and therapy services.
- Make the initial evaluation visit as applicable and re-evaluate each patient’s nursing needs on a regular basis.
- Provide nursing services, treatments and diagnostic procedures requiring specialized skills and assuming full responsibility for each of the patients.
- Participate in continuing education programs, as well as training, and teaching other nursing personnel.
- Develop and implement the treatment plan for each patient under the direction of their physician. Makes referral appointments as necessary for patients.
- Observe, document and report symptoms, reaction to treatments, drugs, and changes in the patient’s physical and emotional condition.
- Acts as liaison between patient, family/caregiver, health insurances and all health care providers.
- Initiate preventive and rehabilitative nursing procedures as appropriate for the patient’s care and safety. Ensure patient’s safety and security.
- Maintain clinical and progress notes for each patient receiving care. Provide progress reports to their physician when appropriate.
- Coordinate services for patients as needed when referring patient to other agencies and health care providers as appropriate.
- Counsel the patient and their family/caregiver in the plan of care and rehabilitation techniques for the patient.
- Document care conferences and outcomes with other health care professionals.
- Direct patient care to include skilled services and home care services to include but not limited to the same duties that an LPN/LVN can perform but to include IV procedures/services that an LPN/LVN can’t perform.
- Recognize emergency situations and be knowledgeable of emergency procedures.
- Be available for on-call duty or be available to other home health staff during all hours that skilled nursing services are provided.
- Complete in a timely manner paperwork necessary to maintain updated patient charts and all mandated forms and documentation as required.
- Adhere to all agency policies, including but not limited to, infection control and safety, education, reporting and practice implementation.
- Draw lab as ordered and report results to physician.
