Position Overview :
As a Lead Program Associate, you will play a vital role in fostering a safe, supportive, and well-maintained living environment for our residents. Working as part of an interdisciplinary team, you will build positive relationships with residents, collaborate across functions to support program staff, and assist management in coordinating property maintenance services. This role is essential to ensuring the effective operation of our supportive housing programs.
Key Responsibilities :
- Develops a positive rapport with each resident through routine contact and communication
- Provides support and assistance by completing care coordination tasks such as; completing referrals for services and residency forms
- Assists in the implementation of case plans and document the client interactions appropriately
- In the absence of the Program Manager or Shift Supervisors serve as the primary liaison between the program and outside service providers, law enforcement, government officials, or other individuals
- Informs Program Manager and all appropriate staff of any program concerns that require resolution or follow up
- Ensure that all paperwork required during an intake is completed and forwarded to the Lead Case Manager or appropriate staff
- Completes incident reports
- Works with case management team to ensure all required service delivery is documented and to identify strengths and weaknesses in data entry and quality
- Responds to immediate client needs, help de-escalate crisis situations, help resolve conflicts and enforce program rules
- Conducts unit inspections to address, coordinate and report pest control and routine emergency,
- Reports maintenance requests and contractor service needs to Program Manager, property owner and housing team
- Completes maintenance request forms and submits it to the Program Manager, property owner and housing team
- Create and distribute maintenance letters and appointment reminders to resident units to assure client prior notification
- Escorts maintenance contractors and vendors to client units to support engagement and unit access
- Oversees the timely and successful completion of maintenance requests
- Maintains, organizes, and updates maintenance records to provide historical data regarding maintenance servicing and property safety
- Inspects property conditions and coordinate maintenance activities between the property
- Completes medication logs for clients and sign off on client medication logs, assist clients in keeping track of medication refills
- Participates in program team meetings, staff meetings, supervision meetings as required and requested per contracts
- Builds and maintains quality working relationships with colleagues, external and internal stakeholders
- Participates in program quality improvement activities that improve program policies, procedures and client and program outcomes
