As part of our ongoing commitment to better serve the Hartford community, we are excited to announce the creation of a Community Liaison Position. This role will play a key part in helping us bridge the gap between clients and the myriad of services available to them. The Community Liaison will be deeply embedded within the Hartford community, working to ensure that clients receive holistic support across a range of services, from housing and transportation to healthcare and education.
Position Summary
The Community Liaison will act as a vital connector between our clients and the broader network of community services in Hartford. They will work directly with individuals and families to identify and address their unique needs, collaborating with local providers to ensure clients receive sustainable, community-based support. This position is integral to NAFI’s mission of ensuring equitable access to resources and helping clients achieve long-term success beyond the services we offer.
Key Responsibilities
- Establish and Foster Relationships: Build and maintain a minimum of 20 formal partnerships with local community providers, including healthcare professionals, school systems, housing services, and other community organizations.
- Client Connection: Ensure 100% of clients are connected to at least one sustainable community organization that can provide ongoing assistance after discharge from our program.
- Community Engagement: Actively participate in a minimum of 5 community groups or initiatives focused on Diversity, Equity, Inclusion, and Belonging (DEI&B) in Greater Hartford.
- Client Education and Resource Navigation: Empower clients by increasing their knowledge of community resources, ensuring 100% of clients report increased awareness upon discharge from the program.
- Post-Discharge Follow-Up: Conduct 6-month follow-up surveys or calls with clients to ensure 80% report continued utilization of community resources and ongoing support.
Required Qualifications
- Local to Hartford: A strong connection to the Hartford community is essential. Applicants should have experience working or living within the area and an understanding of the challenges faced by local families.
- Bilingual Strongly Preferred: Fluency in both English and Spanish is strongly preferred to ensure effective communication with our diverse client base.
- Relationship Building: Experience in developing and maintaining professional relationships with community organizations and service providers.
- Strong Communication Skills: Ability to communicate effectively with diverse populations, both in person and in writing.
- Commitment to DEI: A passion for promoting diversity, equity, inclusion, and belonging, with experience working in organizations or groups that focus on equity-based community engagement.
- Experience in Social Services: Previous experience in social work, community outreach, or case management is a plus.
