Job Summary
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The FNP will provide comprehensive care to patients across all age groups, focusing on health promotion, disease prevention, and management of chronic conditions. This role requires a strong understanding of medical terminology and the ability to perform various clinical procedures. The ideal candidate will have experience in both trauma care settings and memory care, ensuring high-quality patient outcomes.
Duties
- Conduct thorough physical examinations and health assessments for patients.
- Develop and implement individualized care plans based on patient needs.
- Perform clinical procedures such as spinal taps and sterile processing.
- Collaborate with multidisciplinary teams in Level I and Level II Trauma Centers to deliver optimal patient care.
- Utilize eClinicalWorks for accurate documentation and management of patient records.
- Provide health coaching to patients to promote lifestyle changes and improve overall health.
- Monitor patient progress and adjust treatment plans as necessary.
- Educate patients and their families on health management strategies, including medication management and preventive care.
Experience
- Proven experience in a clinical setting, preferably with exposure to memory care or trauma environments.
- Familiarity with medical terminology is essential for effective communication within the healthcare team.
- Experience with eClinicalWorks or similar electronic health record systems is highly desirable.
- Ability to perform dialysis procedures is a plus but not required.
- Strong interpersonal skills to foster positive relationships with patients, families, and colleagues.
