What are the responsibilities and job description for the Help Me Grow Home Visitor position at Neighborhood Alliance?
Help Me Grow is Ohio’s evidenced-based parent support program that encourages early prenatal and well-baby care, as well as parenting education to promote the comprehensive health and development of children by providing in-home services consistent with Neighborhood Alliance, Help Me Grow and Healthy Families America policies. This position is a hybrid position with some in office work and the majority of the work being done remotely or in the field.
Essential Job Functions:
- Provides home-based parent education, advocacy and resource linkages to families of young children prenatally to the time of graduation of the home visiting program, throughout Lorain County
- Conducts visits in the child/family home or other natural environment(s) where the child participates in his/her daily routines
- Administers educational, developmentally-focused home visit lessons and activities
- Facilitates and participates in the development, implementation, review and monitoring of the Home Visitation Family Plan for each assigned child
- Improves child health, development, and readiness by conducting on-going screenings/tools of each enrolled child’s development and health.
- Improves parenting confidence and competence
- Informs, monitors and facilitates the timely delivery of services to the family
- Increases family connectedness to community and social supports while communicating with local agencies and providers.
- Facilitates a transition plan to meet a families individuals needs.
- Maintains and provides accurate and timely statistical data, records and reports.
Necessary Education and Training:
- Professional Degree, Bachelor’s Degree (Preferred) in Social Work or a related field
- 2 years positive work experience with children and/or families preferred
- Experience with low income individuals and families encouraged
- Culturally competent
- Willing to complete program provided training in Growing Great Kids curriculum and Healthy Families America model
- Bilingual (English/Spanish) helpful
- Driver’s License and use of a vehicle for daily travel within assigned area
