FNP Job description-Position Available ASAP:
Private practice clinic in Elbridge, NY. We are currently seeking an FNP to join our team. Join an Integrated, Innovative, Independent Provider Lead Health Care System with Excellent Quality of Life. This is a part time health system employed Family Nurse Practitioner position with a competitive base salary and incentives. Essential Job Functions/Responsibilities:
Perform clinical duties in accordance with the scope of practice per FNP license and in accordance with NYS laws and regulations. Conduct all assigned clinical procedures for the comfort and well-being of patients, per practice protocols. Diagnosing and treating health conditions, formulating treatment plans for acute and chronic diseases, educating patients on lifestyle habits, ordering, and analyzing diagnostic tests, Counseling patients on test results and medications, prescribing medications, conducting examinations of the body, managing overall patient care., Educating patients on preventative care and disease management. Treatment of chronic medical conditions, Acute care for short-term illness, Immunizations, Hypertension, Diabetes with and without complications, Acute Pharyngitis, Long Term Anticoagulants, Various screenings, Chronic pain evaluations and Opioid Dependence evaluations to then send to appropriate physicians in practice.
- Provides comprehensive preventive care and health assessment/screenings, diagnosis, treatment, or referral services to individuals and families
- Assesses individual needs; physical exams of all ages, evaluations for well adults, anticipates problems; follows up on concerns
- Leads and participates in multidisciplinary health care team to develop, implement, and evaluate individualized treatment plans
- Recommend diagnostic testing and procedures; presents findings and recommendations to referring physicians upon request
- Consults with physicians or specialists regarding patient treatment plans at a level of complexity that exceeds the Family Nurse Practitioner’s scope of practice. Takes initiative to engage patients and other health professionals.
- Participates in case of management activities when appropriate
- Prepares and operates a wide variety of medical equipment, including EKG, pulse oximetry, and administers other medications when necessary
- Completes records on admission, transfer, and discharge; maintains all required records in accordance with clinic policies
- Practices evidence-based medicine and adhere to the American Nurses Association Code of Ethics, demonstrates sound judgment in anticipating problems or adverse outcomes; takes appropriate corrective action
Family Nurse Practitioner Requirements:
Board Certification, NYS licensure and DEA required
Extensive knowledge of the health and aging process
Demonstrates practical interpersonal communication skills
Brings an innovative approach to work, can “think outside the box.”
Takes a team approach in accomplishing treatment goals
Possesses strong clinical, interpersonal, and communication skills
Graduation from an accredited FNP program
Professional and ethical behavior
eClinical Works EMR experience a Plus
Knowledge of NYS Prescription Monitoring Program I-Stop (PMP)
