What are the responsibilities and job description for the Clinical Pharmacist - Pharmacy position at New London Hospital?

  • GENERAL SUMMARY
    • Engages patients and healthcare professionals to efficiently and effectively maximize therapeutic outcomes and is responsible for one or more of the following tasks: dispenses medications, monitors patients' drug therapies, works under collaborative practice agreement or per protocol, and provides consultation and education to patients and other staff.

  • FUNCTIONAL DUTIES AND RESPONSIBILITIES
    • Clinical and operational duties

      • Gathers a complete medication history and reconciles medication lists as applicable and appropriate.
      • Completes a comprehensive medication review to make evidence-based recommendations to ensure medications are necessary, safe and cost effective.
      • Identifies and records suspected adverse drug reactions, adherence gaps to medications and barriers to patient adherence.
      • Evaluates medication orders for accuracy, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibilities. Provides age/developmentally appropriate patient care, incorporating patient age when assessing response to medications.
      • If not practicing under a collaborative practice agreement or an approved protocol, contacts providers when there appears to be a possibility of unsafe or inappropriate medication use due to factors including, but not limited to: allergy, medical condition, drug interactions, dosing, and method of administration. Accurately documents all clinical activities and interventions in the electronic record.
      • Serves as a consultation resource to medical professionals and answers inquiries on drug therapy and drug products.
      • Consults patients and caretakers on all aspects of medication therapy when necessary. Assists in discharge planning when requested. Counsel’s patients and caretakers on proper drug administration, potential drug side effects, interactions and precautions. Helps locate medication supplies not generally available.
      • Contributes to the development, maintenance and management of medication related guidelines defined by protocol or collaborative practice agreements
      • Detects and reports medication errors and near misses.
      • Prepares and delivers presentations to facilitate learning or disseminate evidence-based best practice to pharmacists, nurses, physicians, other healthcare professionals in the hospital or ambulatory/outpatient settings.
      • Communicates effectively between departments and teams to ensure optimal patient outcomes.
      • Actively participates in all pharmacy and organizational programs and initiatives to improve patient quality of care, medication compliance, medication adherence and overall efficiency.
      • Pursues and maintains certifications necessary to remain compliant with department accreditations and standards.
      • Prepares compounded sterile preparations, including chemotherapy, as needed, according to USP guidelines.
      • Conducts weekly patient workup of chemotherapy regimens to evaluate for dosing accuracy, appropriate supportive care, drug interactions, and patient side effects. Makes recommendations to oncologist as needed to optimize oncology therapy.
      • Verifies medication orders for chemotherapy and supportive care medications to ensure accuracy and safety.
      • when necessary, supports management of pharmacy inventory, including placing replenishment orders for medications from the pharmacy wholesaler or other vendors in an accurate and cost-effective manner.
      • Performs technician duties, as needed.
      • Documents activities to provide information needed to meet standards, regulations, laws and/or other admnistrative requirements.  Documentation includes controlled substance records, medical profiles, patient care interventions, quality  improvement activities, adverse drug reactions, medication events, and medication reconciliation.
      • Checks all medications before delivery, and works to reduce the potential of medication errors.
      • Ensures readiness for regulatory and accreditation surveys.
      • Participates in the quality improvement and medication use review activities of the department.
      • Follows department procedures and guidelines for the receipt, storage, security, dispensing and disposal of controlled substances.
      • Performs other duties as assigned.
  • POSITION REQUIREMENTS 
    • Knowledge / Skills / Abilities
        • Ability to work independently with minimal direct supervision, with the ability to set priorities and make critical decisions.
        • Effective oral and written communication skills,
        • Strong interpersonal and team skills.
    • Experience
        • 1-3 years’ experience required.
        • Hospital pharmacy experience preferred.
    • Education
      • Graduate of an accredited school of Pharmacy.
      • Current NH pharmacist licensure required
