Responsibilities:
- Works closely with intake staff to coordinate care for patients.
- Provides professional nursing care by utilizing all elements of the nursing process.
- Providing complete skin and wound assessments each visit
- Regularly re-evaluating patient and family/caregiver need.
- Participating in revising the plan of care as necessary
- Initiates appropriate preventive and rehabilitative nursing procedures in relation to wounds.
- Administers medications and treatments as prescribed by the provider in the provider’s plan of care as relates to wounds.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
- Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
Additional Responsibilities:
- Ensures that arrangements for equipment and other necessary items and services are available.
- Supervises ancillary personnel and delegates responsibilities when required.
- Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
- Fulfills the obligation of requested and/or accepted case assignments.
- Actively participates in quality assessment performance improvement teams and activities.
Note: As an LVN, you will not be responsible for care planning/Oasis charting, certifications, or admission visits.
Requirements:
- Must be willing to travel extended distances around the listed position area.
- Valid LVN/LPN license.
- Most hold board certification.
- Commitment to providing high-quality wound care.
- Must be willing to travel for initial training (out-of-state travel required).
Position Details:
- Role: Licensed Vocational Nurse
- Commitment: Full-time (40 hours/week)
- Patient Load: Full-time max census is 30-35 patients (7-9 per day)
- Compensation: $64,000 per year
Training:
Providers will participate in a 3-week long training process to ensure overall preparedness and confidence before seeing patients independently.
- Week 1: Virtual orientation and EMR training.
- Week 2: 4-5 days of in-person training with one of our Advanced Wound Care Trainers (out-of-state travel may be required depending on location).
- Week 3: 4-5 days of shadowing an Advanced Wound Care Provider in an area more local to you.
