Job Type Full-timeDescriptionThe Community Health Worker provides care coordination services and works as part of a collaborative team with Patient Access, Health Information Management, and clinical staff to ensure smooth continuum of care for identified patients, many of which are high risk. They work directly with patients and the Health Center's primary care providers to coordinate patient care and services, which may include involvement of family and friends of the patient. This includes customer service, making follow-up appointments, referrals / referral follow-up and documentation, telephone calls to patients, ensuring patients can make scheduled appointments including providing transportation to appointments, employing enabling services as needed, and directing inquiries; as well as documenting all activities within the Health Center's Electronic Health Record (EHR) or other health information systems. The Community Health Worker also supports efforts to close gaps in care. The Community Health Worker will work closely with the Patient Access Manager and the Quality & Compliance Director in identifying patients with open gaps in care. The Community Health Worker must be able to meet with patients where they are at; to include in the home or in the hospital visits. The Community Health Worker must be able to work in a fast-paced environment, be self-motivated, and be able to prioritize and efficiently manage multiple assignments. The Community Health Worker also serves as the point of contact, advocate, and informational resource for patients, care team, family / caregiver(s), payers, and community resources. RequirementsBachelor's degree in health or social science field with at least 1400 hours of experience working as a Care Coordinator / Community Health Worker in a paid or volunteer role within the past three years, or at least 20 hours of CDC-endorsed CHW Core Consensus Competencies (C3) training. Be knowledgeable in medical terminology and computer skills. Current Basic Life Support certification or is willing and able to obtain. Skills to include teamwork, knowledge of patient care, social determinants of health, and patient compliance issues. Excellent interpersonal and verbal communication skills. Skilled with computers and computer applications. Strong attention to detail and possesses problem resolution skills. Positive attitude and high energy, is a quick learner, is adaptable to change and is resourceful and proactive when identifying and resolving problems. Valid driver's license and means of personal transportation; position may include driving patients with NCFHC vehicles. Salary Description Starting at $21.11 / hr.
