What are the responsibilities and job description for the Facility Nurse Navigator position at NueHealth?
The role will manage the surgical patient preparation for surgery and post op education in the stay suites. Individuals will assist in the screening and assessments of patients based on Preop history and physical and lab work. Individuals will coordinate care with physicians, ensure communication and preparation is streamlined. As needed will provide patient specific education on diagnostic testing, diagnosis, and treatment plan and escalate concerns to Anesthesiologists/and or surgeon.
Maintains excellent communication and follow up with the patient. Establishing and maintaining relationships with providers. Oversee and aid with discharge planning, coordinates patient’s discharge needs with additional services as necessary.
Qualifications:
- 3 years RN experience in a Hospital and/or ASC
- 2 years clinical orthopedic experience
- Active RN license as required by State
- BLS certification
- Excellent interpersonal skills and written and oral communication required
Essential Functions:
- Coordinates with preadmission testing/PAT and Anesthesia/Primary Care Physician and/or surgeon to ensure appropriate candidate for out patient setting based on patient’s medical conditions prior to surgery.
- Collaborates with members of the medical team in the ASC, Suites and Surgeons’ offices.
- Acts as a liaison between the patient, patient’s family, physicians, nursing, and ancillary contracted services.
- Reviews pertinent information about the patient’s medical, psychosocial, functional status.
- Works within criteria set by the pre-admission/registration process; sends appropriate referrals to anesthesia/surgeon for final determination when information warrants.
- Provides necessary information and education to the patient and support person for the following: pre-operative, intra-operative, discharge and post discharge processes.
