QUALIFICATIONS:

1. Graduation from an accredited school of professional nursing. RN license strongly preferred.

2. Currently licensed as a registered or practical nurse with the Indiana Professional Licensing Agency. RN license strongly preferred.

3. Supervisory experience in the care of older adults and knowledge of Indiana Residential Care Facility rules are desired.

4. Must be able to:

A. Set priorities and organize supervisory duties.

B. Utilize problem-solving skills.

C. Communicate effectively and maintain good working relations with personnel in all facility departments, other health-related individuals, and community agency personnel.

5. Must be willing to:

A. Work the floor and fill in as necessary.

B. Keep abreast of new trends in nursing and dementia care by reading current journals and publications of the long-term care industry.

C. Attend professional meetings, workshops, and conferences on dementia and long-term care industry issues, as approved or requested by the supervisor.

ADMINISTRATIVE RESPONSIBILITIES:

1. Plans, develops, organizes, implements, evaluates, and directs health care services in accordance with applicable rules, regulations, and physician’s orders to ensure the needs of each resident are met. In collaboration with the Administrator and the Medical Director, implements, enforces, and periodically updates written policies and procedures concerning resident assessment and care, admission/discharge planning, physician and nursing services, and the administration, storage, and control of medications and supplies.

2. Develops and periodically updates health services objectives and statements of philosophy.

3. Develops methods or coordination of nursing services with other resident services to ensure the continuity of the residents’ total regimen of care.

4. Selects and recommends for appointments the members of the health services personnel defines the respective duties, and establishes the general pattern of delegated authority and responsibility.

5. Maintains and periodically updates written job descriptions of each level of nursing personnel.

6. Administers the personnel practices under the polices established by the governing body of the facility.

7. Performs administrative duties such as providing oral or written reports, recommendations, evaluations, studies, medical forms, clinical record documentation, etc. as necessary and/or as requested by supervisor.

8. Maintains a current reference library of nursing resources that will make sure nursing services be met acceptable for nurse practice acts and applicable regulations.

9. Implements and maintains a dynamic Quality Improvement program within the nursing department in coordination with the facility-wide Quality Improvement plan.

10. Communicates to the Administrator and when appropriate, the attending physician, the status of the residents, the occurrence of incidents and accidents, and unresolved administrative problems within the Nursing Department.

11. Arranges for and participates in programs of in-service education, including the orientation of new members of the nursing staff.

12. Delegates’ authority to a qualified licensed nursing staff member for responsibility of the Nursing department in his or her absence.

13. Participates in surveys conducted by authorized state agencies. Assists in preparation of written corrective action plans to state deficiencies.

14. Recommends methods and techniques for resource-sparing utilization of nursing supplies, equipment, and staff time.

15. Participates in preparation and allocation of nursing department’s budget.

16. Manage and maintain monthly budget for IHCC documentation, billing submission, and RN authorization.

COMMITTEE FUNCTIONS

1. Serves on, participates in, and attends various committee meetings of the facility. This shall include, but is not inclusive of all, the following: Quality Improvement, Policy and Procedures, Infection control, and Safety committees.

2. Provide written and/or oral reports for health service programs in activities as required, or as maybe directed by such committee(s).

3. Schedules, attends, and participates in developing the agenda for nursing team meetings as necessary.

4. Participates in administration/ discharge planning meetings.

5. Evaluates and implements recommendations for established committees as they pertain to nursing departments.

PERSONNEL FUNCTIONS:

1. Recommend staffing patterns and mix by education and skill sets to adequately provide quality health service and care.

2. Recommends the number in type of nursing personnel needed to fulfill established staffing patterns and to meet the scheduled and unscheduled needs of the residents.

3. Assists the administrator and/or human resources in the recruitment, interview, selection, hiring, evaluation, and termination of nursing personnel.

4. Is responsible for ensuring adequate number of professional, licensed nurses are scheduled on each shift to provide quality care and services on each unit.

5. Is responsible for ensuing an adequate number of qualified medication aids and/or certified nursing assistants are scheduled on each tour of duty to provide quality care and services on each unit.

6. Oversees the development of nursing staff assignments, tour of duty hours, and task distribution.

7. Delegates to Nursing Staff the administrative authority, responsibility, and accountability necessary to perform their assigned duties.

8. Implements, enforces, and periodically updates written procedures ensuring that nursing staff including private duty and/or per diem staff, have valid and current licenses or certifications as required by state licensing and registry agencies.

9. Oversees and monitors care delivery by nursing staff through daily observations and continuous communication with supervisory staff to ensure performance of work assignments in accordance with professional standards of care.

10. Monitors absenteeism to ensure that an adequate number of nursing staff are on duty at all times.

11. Reviews complaints and/or grievances made or filed by departmental personnel.

12. Prepares oral or written reports regarding staff competency or performance as required by facility policy or requested by the Administrator.

13. Ensures that disciplinary action is administered in accordance with facility policies and without regard to race, color, creed, national origin, age, sex, religion, handicap, or marital status.

HEALTH CARE FUNCTIONS:

1. Participates in assessment and selection of residents for admission to the facility.

2. Participates in the coordination of each new resident admission to ensure the nursing personnel are informed of the resident’s expected arrival time, unit assignment, and that the unit is ready.

3. Ensures that physicians’ orders are available for each resident upon admission.

4. Completes initial and on-going resident assessments as required by state regulation and in accordance with facility policy.

5. Completes and updates resident service plans in accordance with facility policy.

6. Periodically reviews clinical records and observes resident care to validate that staff are providing the planned interventions for each resident, and to ensure that professional standards of nursing practice and facility policies are maintained.

7. Oversees compliance with all applicable licensure regulations relative to the Nursing department.

8. Assists physicians during rounds as necessary.

9. Actively participates in maintaining clinical records in accordance with state regulations specific to Physicians’ Services.

10. Completes daily rounds throughout the facility to observe residents and to ensure residents’ needs are met.

11. Monitors medication passes and treatment schedules to ensure that services are provided as ordered and in accordance with facility policies and procedures.

12. Participates in development and implementation of individualized service plans for each resident and coordinates care delivery with disciplines and department to ensure the highest quality of resident’s services.

13. Conducts routine meetings with licensed and certified nursing staff to formulate policies, plan programs, and integrate various resident care services.

14. Directs and reviews the periodic performance evaluations of Nursing Staff to determine conformance with job descriptions, facility policy, and practice standards. Completes performance evaluations of employees under his/her direct supervision.

15. Establishes and maintains professional and supportive relationships with residents and their family members, and with allied professional community services.

16. Provides direct care and other tasks relative to nursing including on-call availability as required to meet the residents’ needs or as requested by the supervisor.

STAFF DEVELOPMENT FUNCTIONS:

1. Participates in the development and implementation of structure in service and education programs for the nursing department to ensure education and training needs are identified, programs are timely and in accordance with applicable policy and regulations, and program content is appropriate to the education and skill sets of the participates.

2. Supervises the education and orientation of nursing staff, interprets principles of good management to supervisory personnel, and encourages staff to further their own educational growth and enhance their skill set.

3. Recommends to the Administrator educational material and professional resources necessary to maintain current knowledge of state regulations and nursing standards of care.

RESIDENT’S RIGHT FUNCTIONS:

1. Assures the nursing staff receives orientation to Resident Rights prior to direct care assignments, and on-going education specific to the rights of each resident.

2. Demonstrates a working knowledge of Resident Rights though exemplary, consistent application of the principles in all aspects of resident care delivery, staff counseling, and ancillary service delivery.

3. Monitors care delivery to ensure that all residents are treated fairly, and with kindness, dignity, and respect.

4. Maintains resident privacy during resident care and confidentiality of all resident records. Monitors staff adherence to these principles.

5. Actively participates in the facility complaint/grievance process by reviewing complaints or grievances related to health care, completing investigations including staff interviews in an attempt to identify deficient practices.

6. Responds to complaints or grievances by recommending corrective action plans to the administrator when substandard Resident Rights practice are suspended or identified.

7. Maintains a record of resident complaints or grievances relative to resident care and services that summarizes the investigation, actions take to resolve the complaint and the effectiveness of these actions.

8. Actively participates in the facility Abuse Prevention Program by directing and monitoring nursing staff orientation and performance to ensure all efforts to prevent resident abuse or mistreatment are in place. Provides investigation summarizes in response to allegations of abuse or mistreatment, and immediately implements corrective actions if such allegations are validated.

SAFETY AND SANITATION

1. Actively participate in facility Safety Program through development, implementation, and evaluation of safety standards and policies.

2. Monitors nursing staff during daily rounds to ensure adherence to safety and sanitation standards and policies.

3. Ensures that all nursing staff work areas, i.e. nursing stations, medication rooms, charts, and utility rooms are maintained in a clean and sanitary manner.

4. Ensures that all nursing staff participated in and conduct all fire safety and disaster preparedness drills in a safe and professional manner.

5. Assists the Administrator in coordination of the activities and recommendations from the Safety and Infection Control Committees into facility-wide Quality Improvement Program.

6. Ensures that nosocomial communicable and/or infectious disease among the residents and staff are monitored for trends, prevention efforts are in place, and reports of these diseases are made to local and state agencies in accordance with all applicable regulations.

7. Develops, implements, and maintains procedures for reporting hazardous conditions or equipment.

EQUIPMENT AND SUPPLY FUNCTIONS:

1. Recommends to the Administrator the equipment and supply needs of the Nursing department.

2. Ensures that adequate par levels of medical treatment supplies and other nursing care supplies and equipment are maintained at all times.

3. Develops and implements procedures for the safe operation of all nursing service equipment.

4. Acts as a liaison between nursing and laboratory services to assure facility policies and procedures regarding diagnostic and routine laboratory tests are adhered to.

STATEMENT OF JOB:

Plans, organizes, directs, and coordinates all health functions within the Nursing department. Ensures the delivery of the highest quality of care and health services to the neighbors in an effective and timely manner while maintaining compliance with applicable regulations and professional standards of nursing care.

DELEGATION OF AUTHORITY:

The CNL has the written administrative authority, responsibility, and accountability for functions and activities of the nursing staff within this nursing facility alone. The CNL assumes responsibility of the Administrator on his/her absence.

This job is similar in nature to a Director of Nursing (DON) in a skilled nursing facility.

Pay: $65,000.00 - $75,000.00 per year
