Certified Home Health Aide
Territory : Wayne County - Southfield, Royal Oak, and surrounding areas
Optimal Care, Inc. is seeking a Certified Home Health Aide (HHA) or Certified Nursing Assistant (CNA) committed to providing the highest quality care in our service area.
We offer competitive visit rates and excellent working conditions. Home care experience is preferred, but not required.
Certified Home Health and Hospice Aide Responsibilities :
Personal hygiene
Ambulation and exerciseReporting changes in the client's condition, including skin conditionNutritional support including preparing meals, meal set up, and assistance with feeding and promoting oral intakeLight household activitiesReading and recording temperature, pulse, respiration, weight, and blood pressureApplies safety principles and proper body mechanics while assisting patient with ambulation, exercise, client transfers, range of motion, position, and light household chores.Prepares and provides adequate nutrition and fluid intake : provides medically prescribed diets under supervision and as instructed in the Aide plan of care.Performs homemaking and other environmental services that facilitate receiving sanitary and safe care at home.Assists patients with reminders for medications that are ordinarily self administered.Observes, reports, and documents changes in patient status or expectations service promptly to a supervisor or other professional.Understands basic elements of body functioning and reports verbally and in writing changes in patient status and body functions as indicated.Recognizes emergency situations and implements appropriate emergency plan and protocols when indicated.Demonstrates good basic infection control and prevention practicesAccurately documents care provided and submits visit records as per agency guidelines.Attends in-service programs according to agency policy. Certification as a home health aide as demonstrated by one of the following :Successful completion of a training and competency evaluation program offered by an allowed home health agency.
Successful completion of a competency evaluation program that addresses all federal requirements.Successful completion of a nurse aide training and competency evaluation program and is found to be in good standing in the state nurse aide registry (CNA).Successful completion of a State administered program that certifies Home Health Aides (HHAs) and that meets or exceeds the federal requirements.Successful completion of the competency skills demonstration evaluation required for home health aide certification by the agency.Takes verbal and written instruction well.Works in a team responsibly and independently without direct supervision.Records and reports observations and activities.Demonstrates good communication skills and ability to read, write, and verbally report.Demonstrates knowledge and skills necessary to provide care to and communicate with primarily the geriatric and adult population.Demonstrates knowledge of the principles of the physical, emotional, and developmental needs of the geriatric and adult population.Able to assess data reflecting the patient's status and interpret the appropriate information needed to identify each client's requirements relative to their age-specific needs.Demonstrates computer skills sufficient to use the electronic medical record effectively.Valid Driver's LicenseCurrent Auto InsuranceReliable TransportationOptimal Care, Inc.
