Job Title: Case Manager
Summary:
This position is within our homeless day center, Street 2 Feet Outreach Center. Working with other team members, the case manager will determine resources needed to meet the individualized needs of the participant. The case manager will take all necessary actions in assuring the implementation of the goal plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps and recommending strategies. A commitment to the RHD values should be demonstrated as job duties are performed.
Essential Duties and Functions
Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective.
Assist in the development of goal plans and reviews in collaboration with the program participant, provide linkage and referrals to community resources, assist in identifying housing options, employment resources, securing vital documents, and providing support and advocacy.
Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.
Direct Client Care
Provide site-based services to program participants focusing on intake/assessment, connection with resources, achievement of goals such as securing shelter/housing and employment.
Administrative Functions
Complete all assessments and documentation as required by RHD, funder, licensing/ accreditation entities.

Qualification
Bachelor’s Degree in Human Services or related field.
- Intermediate computer skills
- One year Case Management experience preferredTwo years' general work experience providing support to individuals in homeless services, behavioral health, Addiction Recovery required.
- Valid driver’s license
Job Types: Full-time, Contract
Pay: $18.00 - $19.00 per hour
Expected hours: 40 per week
