As the Social Services Worker, your responsibilities will include:
- Working with residents to identify their psychosocial, mental, and emotional needs
- Work with residents and family members to find community resources
- Review and set policies concerning resident care and quality of life
- Discharge planning
- Complete Social Services MDS reporting
- Care planning and assesments
- Resident Advocate
- Interdisciplinary team meetings
