Pomona Community Health Center dba ParkTree Community Health Center (ParkTree) is a non- profit, Federally Qualified Health Center (FQHC) whose mission is to be the medical home for the underserved in our community by providing high quality preventative and primary care health services.
Job Summary: Outreach and Enrollment Specialist are responsible for enrolling patients within the health center completing financial screening, sliding fee scale applications, and providing education and guidance for patients regarding program eligibility and services provided by PCHC. Outreach and Enrollment Specialist are responsible for engaging with newly assigned managed care patients, and vetting assignment rosters for accuracy. Conduct consistent audits on program applications and maintain valid records. Outreach and Enrollment Specialist will assist patients with initial appointments to establish care with appropriate clinic location based on payer, program or geographic needs of the patient. Outreach and Enrollment Specialist may be required to attend community events and conduct effective outreach to new potential patients and ensure productivity standards are met.
Essential Duties:
1. Initiating contact with newly assigned managed care patients, initiating appointments to establish enrollment within the health center.
2. Complete initial patient paperwork required such as; financial screening documents, sliding fee scale application, other program applications.
3. Ensure accurate and complete payor set up within the patient’s chart are completed upon enrollment within ParkTree Community Health Center.
4. Provide educational guidance for both patients and staff regarding programs and services provided at Parktree Community Health Center.
5. Maintain accurate documentation and tracking of all enrollment efforts including to but not limited to; meeting scheduling metrics set by Director of Revenue Cycle Operations, maintaining current records of managed care rosters, reporting outcomes on a weekly/monthly basis to Director of Revenue Cycle of Operations.
6. Collaborating with all departments to ensure quality goals are met as defined by leadership.
7. Advocate for patients by assuring the best use of program eligibility defined by services provided by specific sites.
8. Maintain current knowledge of various program/payor requirements and eligibility guidelines as well as reporting updates and changes of all accepted programs to appropriate departments and department leaders.
9. Engage with all patients and staff in professional demeanor, remaining a good steward of company resources identifying and reporting areas of improvements to leadership in a timely manner.
10. Adhere to HIPAA regulations maintaining patient confidentiality at all times.
The aforementioned services will only be performed at a location in ParkTree’s approved HRSA service sites as listed on Form 5B and 5C.
Job Qualifications and Skills:
1. Bilingual in both English and Spanish required
2. High School Diploma or equivalent required
3. Valid California driver’s license, ability to provide proof of insurance is required
4. Ability to work with diverse patient populations in a respectful manner
5. Knowledge of EHR/EPM systems, basic computer skills including basic understanding of MS Word, Excel and Outlook preferred NextGen experience
6. Knowledge of Medi-Cal Managed Care plans, and various Medi-Cal programs preferred
7. Understanding of all contracted payors, including Medicare and Medicare Managed Care plans preferred
8. Ability to understand and maneuver various payor portals
9. Develop professional relationships with provider representatives of contracted payors
10. Possess effective communication skills, both written and verbal formats
11. Possess a strong level of professionalism
12. Strong critical thinking and problem solving skills
13. Effective time management skills
14. Ability to effectively manage competing priorities and meet deadlines as requested
15. Strong customer service skills
Physical Requirements:
1. Prolonged periods of sitting at a desk
2. Prolonged periods of typing and working on a computer
3. Must be able to lift up to 30 pounds at times

Pay: $21.00 - $21.50 per hour
