What are the responsibilities and job description for the Care Manager position at Partnership for Children?
We are looking for exceptional individuals who are motivated and committed to providing outstanding service each and every day to children and families in Essex County.
Key responsibilities include:
- Utilize a team-based approach in facilitating Child and Family Teams and meetings
- Provide face-to-face contacts with youth and families in the home or community on a monthly basis, in addition to ongoing telephone contact
- Provide crisis on-call coverage for PCE as needed (PCE provides 24/7 on-call crisis services for all PCE clients)
- Coordinate and facilitate comprehensive assessments of strength and needs of children and families in their community
- Serve as a collaborative partner in the implementation and development of ISP’s (Individual Service Plans) for children and their families utilizing the wraparound model of care
- Coordinate Care Management across multi-systems
Qualifications and Skills:
- BA in a related field, such as social work, counseling, psychology, criminal justice or special ed., and a minimum of one year related experience. MA in related field preferred.
- Professional experience as a Social Worker, Case Worker, or Case management background in Human Services
- Experience with community relations and resource development are preferred.
- Bilingual in Spanish, Portuguese and/ or Creole a plus
- Prior experience in managing wrap-around services a plus
- Must possess excellent writing skills
- Must be computer literate
- Knowledge of the Essex County community preferred
- Must possess a valid driver’s license
