GENERAL DESCRIPTION OF POSITION
Provides technical assistance and support of Consultation services received by clients. Provides training to staff. Monitors positive behavior plans. Provides crisis intervention via ARKSTART.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Responsible for developing, screening, and assessing therapeutic treatment plans.
2. Responsible for assisting in the design and integration of individual objectives as part of the overall individual service planning process.
3. Provides training to appropriate staff and family members concerning positive behavior plans and technical assistance regarding special community living services strategies as identified in the person's service plan.
4. Provides necessary reports and attends selected individual staffing.
5. Provides necessary reports and attends selected individual staffing.
6. Provides information and assistance to persons responsible for developing the person's overall service plan.
7. Consults with and provides information and technical assistance with other service providers or direct service staff or family members.
8. Screens, assists, and develops positive behavior support plans, assisting staff in implementation, monitoring, re-assessment and plan modification.
9. Assists in securing guardianship and provides annual reports to the courts.
10. Research an individuals eligibility for social security and benefits and assist with application if appropriate.
11. Assists in appeals to Social Security when individuals are denied benefits.
12. Prepare informational/training materials for consumers and staff related to the legal rights and needs of individuals with disabilities.
13. Assists in the preparation and review of agreements/leases with other providers, governmental entities, contact employees, landlords, etc.
14. Assist in the review and preparation of responses to EEOC and legal complaints against Pathfinder, Inc.
15. Assists in reviewing client related procedures and forms to insure that individual rights are protected and their responsibilities are clearly outlined.
16. Provide Crisis Intervention services of training, establishing linkage between providers to best serve client, meeting providers/staff/parents, etc., Review files, Draft and maintain comprehensive service evaluations, draft and maintain cross system crisis plans, rotate on call responsibilities with other START coordinators, assist with crisis intervention when they occur, attend training and meetings as required.
17. Perform any other related duties as required or assigned.
