Pathways to Housing DC (Pathways DC) is an innovative agency based in the heart of Washington, D.C. We believe that housing is a basic human right, and that without housing people are unable to address their mental health disorders, addictions, and other life stressors. We provide each person entering our program their own apartment, without requiring treatment compliance or sobriety as a prerequisite. We then provide each person with intensive support services to help them stay housed and begin their journey to recovery.
Under the direction of the Associate Director of Homeless Operations, the DDSC Lead Case Manager is responsible for the daily operation and supervision of DDSC housing services. The Lead Case Manager will help coordinate a team of case managers, ensuring effective service delivery. They will be responsible for assuring that the agency values and philosophy of consumer choice and respectful concern are expressed in all aspects of team practice. They will serve as the liaison for case coordination with Pathways and other providers to ensure timely and accurate sharing of information. This role requires strong collaboration and leadership skills, as well as a deep understanding of homelessness issues and the ability to engage with vulnerable populations.
Responsibilities:
Co-Lead Coordination
- Collaborate with Associate Director of Homeless Operations to ensure coordination and operation of case management team
- Share responsibilities for team management, training, task assignments, and performance evaluation.
- Collaborate with the Clinical Supervisor on policy and procedures.
- Manage day to day operations of the program when the Clinical Supervisor is absent or unavailable
- Supervise electronic and written record-keeping activities, e.g., HMIS progress notes, vital record orders, data entry, service plans, incident reports and client profiles
- Foster a collaborative and supportive team environment, promoting open communication and information sharing
Case Management/Housing Services:
- Provide case management services to a small caseload of clients as needed and as determined in collaboration with the Clinical Supervisor
- Plan and implement housing services, in collaboration with the DDSC team members
- Coordinate housing services of the DDSC team with DHS outreach, other providers, and social services
- Collaborate with other agencies to eliminate barriers for individuals to receive necessary services and housing
- Monitor waiting lists, matching lists, etc. in order to track client progress towards housing
- Represents Pathways at meetings with other providers, conferences, and at community meetings as assigned by the Associate Director.
- Represent Pathways and the DDSC team at conferences, community meetings, and other venues
- Generate monthly reports on progress, successes, and challenges; monitor outcomes and analyze team metrics
- Provide other related duties as required.
Qualifications
- Bachelor’s Degree in Social Work, Psychology, Sociology, Counseling, or related social service/science or healthcare related discipline required.
- 3 years’ experience in case management and/or counseling services to individuals/families experiencing homelessness or other related populations.
- Experience as Team Lead or Supervisor is strongly desired.
- Excellent interpersonal, communication and writing skills, as well as computer literacy.
- Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations.
- Bilingual English/Spanish is strongly desired.
Hours: 40 hours per week; 8am-4:30pm
Salary: $63,000-$65,000 before deductions and withholdings
