The Residential Program Coordinator/ DDP's goal is to provide growth and business development for the agency in community care with an emphasis on the, elderly, Developmental Disabled and Mentally challenged population. The right team member MUST have knowledge of the HOME CARE INDUSTRY FOR THE ELDERLY AND DD POPULATION, PRIVATE PAY MARKET AND THE MEDICAID WAIVER PROGRAMS.
Specific Requirements
The right candidate should have a background in LPC, DDP, DSP, Nursing, social work, behavioral therapist, care coordination, Community case management, Support co-ordination, IDD agency program director.
· Highly organized, strong attention to detail and ambitious in getting the job done.
.Responsible for quality assurance and agency compliance
.Responsible for shaping the direction and growth of agency.
· Experience working with CCSP/SOURCE/ICWP/NOW AND COMP. GAPP waiver programs a must. Also workman's COMP and other skilled home care programs.
· Able to travel.
· Must possess strong customer service skills and have community connections to aid in business development
· Must demonstrate excellent oral and written communication skills and the ability to listen effectively
· Must have the personal integrity and ability to discreetly handle confidential information
· Must possess demonstrated excellent computer literacy and Microsoft Office skills, social media skills
· Must possess the ability to excel in a team environment as well as work independently with little supervision
· Must possess the desire to serve clients and coworkers with exceptional customer service
· Must possess strong interpersonal skills with the ability to effectively communicate with a diverse workforce

JOB DUTIES
-Reponsible for agency QA and compliance and performing all DDP duties per DBHDD policy, agency day to day operations
-Complete and update all HRST
-Supervises the delivery of service and ensures the strategies reflect the goals and objectives of the ISP.
-Supervises high intensity services that address health and safety risks for the participants that include reviewing, writing, implementation and effectiveness care plans.
-Involve in identifying ongoing supports as needed (medical and behavioral) in collaboration with appropriate personnel.
-Follow up on all state nurse's recommendations and HRST trainings.
-Complete incident report and submit it to DBHDD
-Attend all ISP meeting and advocate for needs and or changes of services.
-Train new employees/ direct care staff on documentation in accordance with DBHDD standards.
-Monitor their behavior, record and collect data on patients' progress, and communicate with clients.
-Write correction plan of action after state audits.
-Oversee the functions of group home and all other services IDD services
-Created monthly reports on each individual in the home.
-Ensured all medications, Prescriptions, Money, and documentation was correct and up to date.
-Kept documentation in compliance with state regulations as well as state reporting.
Ensured proper care and safety was being provided for all individuals.
-Monitored the implementation of goals and objectives during the delivery of services
· Ensuring consistent accuracy of database and file information for both caregivers and clients
· Creates and maintains staff schedules, providing coverage for call outs.
· Maintains staff and client database (e.g. employee assessments, dates and reminders of TB, client admission, client hospital admission and discharges
· Maintains strict confidentiality on patient, agency, and employee matters.
· Participates in the Performance Improvement process
