As a patient care coordinator, you will have the opportunity to serve a wide variety of patients, from first phone call to the friendly face behind the desk, all while assisting in managing their cases, we recognize that you play a huge part in the care team!

POSITION: PRN/Part-time patient care coordinator to serve our patients from first phone call to final appointment at Peak Motion Physical Therapy in Sundance, Wyoming
WORK SCHEDULE: As needed to 8 hours/week (flexible, no weekends)
RESPONSIBILITIES AND DUTIES:
· Manage and support an excellent, peak patient experience from day one.
· Manage the patient intake process, ensuring forms are completed, scanned, and updated.
· Check for accuracy of insurance status, eligibility, account information, and all vital statistics.
· Complete referrals and pre-certifications in a timely manner.
· Assist provider communication requirement with patients and referring physicians.
· Support clinic waiting room, gym, and exam rooms remain clean and orderly.
· Support marketing efforts including provider drop-ins to assist in fostering good relationships with referring, and potential referring, providers.
MINIMUM QUALIFICATIONS:
· Associate's degree
· 4 years of experience in an office environment preferred (qualified new graduates welcome to apply)
PERSONAL QUALITIES:
· Possession of effective work strategies including flexibility, good organization and communication skills, time management, ability to work independently and prioritize multiple work tasks.
WORKING CONDITIONS AND OTHER QUALIFICATIONS:
· The employee is regularly required to stand, walk, bend, crouch, sit, talk, and listen.
· The employee must be able to lift and/or move up to twenty pounds.
· CPR and First Aid certification desirable (not required).
