What are the responsibilities and job description for the Pediatric Home Health Nurse- LVN/RN position at Pediatric Private Duty Nursing?
SCOPE OF POSITION:
Provides skilled care to clients in their residence. The LVN is supervised by a Registered Nurse or the Supervising Nurse.
QUALIFICATIONS:
1. Graduate of an accredited school of nursing, currently licensed by the State of Texas to practice
2. One (1) year experience providing comprehensive skilled nursing care in the home health environment preferred
but not required.
3. Six months experience in caring for clients [experience may be in any healthcare setting]; OR will be required to
pass a written test OR must have any specialized skills observed as satisfactorily performed by the Supervising
Nurse or designee.
DUTIES/RESPONSIBILITES:
1. Provides comprehensive skilled nursing care in accordance with the plan of care, and within the scope of LVN
practice to clients in their residence.
2. Administers medication(s) in accordance with the plan of care, physician orders, and agency policy.
3. Reports all changes to the plan of care or care plan to the RN or Supervising Nurse prior to implementation,
obtaining RN countersignature on all verbal orders received.
4. Accurately documents client signs, symptoms, responses to care, and care given. Submits clinical notes in
accordance with agency policy.
5. Participates in patient case conferences and other professional meetings as directed. Interacts with the
interdisciplinary care team to coordinate patient care.
6. Assists in developing and implementing discharge plans for agency clients in accordance with agency policy.
7. Drives to and from client residences to perform duties.
8. Performs other agency related duties as assigned.
PHYSICAL REQUIREMENTS:
Visual/hearing ability sufficient to comprehend written/verbal communication. Must be able to work eight (8) hours per day
minimum while driving, standing, sitting, bending, reaching, and using office equipment and information systems. Must be
able to lift and transfer patients.
