Title : Nurse Practitioner - Palliative Care (Blingual)

Salary : $115K-143K plus $15,000 Sign-on Bonus or Student Loans Assistance, paid over the course of 3 years
Responsibilities :
Conducts comprehensive assessments.
Performs physical examinations.
Orders appropriate diagnostic testing
Formulates a problem list
Recommends interventions consistent with a care plan that advances the objectives of palliative care
Provides interventions as appropriate, consistent with a care plan that advances the objectives of palliative care
If appropriate, takes the lead in implementing key elements of palliative care, including family meetings, goal setting discussions, and symptom management.
Performs hospice eligibility review and engages in accurate hospice-related discussions
Collaborates with patients’ physicians and other treatment team members
Evaluates patient's response to treatments, modifies or changes when needed.
Provides teaching and guidance to family members.
Offers choice when recommending hospice and assists in handoff to hospice providers if hospice election is pursued
Performs handoff to other members of the treatment team consistently and accurately to ensure continuity of care Recognizes emergent or urgent situations requiring escalation and take appropriate action Requirements :
Master’s Degree in Nursing; graduate of accredited Nurse Practitioner program required.
License / Certification : New York State RN licensure, Certified Nurse Practitioner
Hospice & Palliative Care certification strongly encouraged by year 2 of hire.
