Key Responsibilities
- Provide direct physical therapy treatment in patients’ homes and assess their home environments.
- Identify and prioritize patient needs for physical therapy services.
- Communicate with physicians regarding patient reactions to treatments or changes in their conditions.
- Assess the needs of patients and their families for additional home health services and make appropriate referrals.
- Engage in the discharge planning process for patients.
- Supervise physical therapy assistants in alignment with organizational policies and state regulations.
- Strong assessment and evaluation capabilities.
- Proficient in executing care plans and administering treatments effectively.
- Excellent communication and collaboration skills with healthcare professionals.
- Familiarity with home health services and community resources.
- Degree from a baccalaureate or master’s program in physical therapy accredited by an appropriate organization.
- Valid license to practice as a physical therapist in the state, if applicable.
- Minimum of two years of relevant experience as a physical therapist, with a preference for community or home health experience.
- Current CPR certification.
- Valid driver’s license and reliable transportation.
