Nurse Care Coordinator Job Description
Philadelphia FIGHT was initially founded in 1990 as a comprehensive AIDS service organization. Today we are a Federally Qualified Health Center (FQHC) with clinical sites that provide state-of-the-art culturally competent primary care, advocacy, social services, consumer education and outreach for patients in need regardless of HIV or insurance status.
The patients and staff at Philadelphia FIGHT are diverse and represent a broad spectrum of race, religion, gender identity, sexual orientation, HIV status, and physical ability. Qualifications: A degree in nursing is required and a Registered Nurse license for the state of Pennsylvania. We are looking for nurses that have a general knowledge in science, medicine, and the running of a medical clinic. Can demonstrate the ability to understand the overall purpose of clinical care and the importance of support staff. Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health counselors. Must be able to demonstrate judgment, patience, integrity, organization, and prioritization in a multi-task environment. Needs to have attention to detail, dependability and flexibility.
Responsibilities include:
Clinical Responsibilities (0.75 FTE)
Administrative Responsibilities (0.25 FTE)
Physical Demands: Occasional lifting of 10-20 lbs. and pushing of 5-20 lbs. Sitting approximately 80% of the time. Normal accessibility and mobility required. Ability to work at a computer screen for long periods of time.
Work Environment: Generally, works in a high-volume, clinical office environment but may occasionally be required to perform job duties outside of the typical clinical setting. May also be required to provide coverage at all Philadelphia FIGHT Community Health Centers.
Salary and Benefits: Competitive salary and benefits.
Other: Initial probationary period is three months. Performance of other tasks may be assigned on an as-needed basis.
Philadelphia FIGHT requires proof of completed COVID-19 vaccination series for all new employees hired. Medical and religious exemptions will be honored consistent with EEOC regulation.
